What the inspector wrote

On 05/15/2025 at 10:00 am, Licensing Program Analyst (LPA), Crystal Green conducted an announced Case Management inspection for a temporary increase in the licensed capacity. Upon arrival, LPA met with Licensee Representative, Turosca Allen. The Licensee, TAC Smart Enterprises, Inc., has requested an individual emergency waiver to temporary increase their facility capacity to assist families affected in the LA Fires. This facility is currently licensed to operate with a capacity of 28 children: (15) preschool children from ages 2 to 6 years and (13) school-age children from ages TK to 12 years old. The licensee has requested to serve (40) children: (4) infants ages 0- 2 years in Room #1, (22) preschoolers ages 2- 6 years Room #2 and#3, and (14) school-age children age TK- 12 years in the school-age portable room. The increase of capacity waiver will allow the facility to operate beyond its licensed capacity temporarily while the State of Emergency for Los Angeles and Ventura Counties is in effect. The facility received an approved fire clearance to operate at the capacity of (40) children. Hours of operation will be Monday to Friday from 5am to 10pm. Present during today\342\200\231s inspection were (23) children with (5) staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Turosca Allen that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. Page 2: LPA continued to tour the facility. No measurements were during this inspection. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Per Ms. Allen, she has (4) cribs on hand to accommodate the requested infant capacity. Drinking water is available in the classrooms via filtered water dispenser and child\342\200\231s personal water bottles. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 5/13/2025. LPA observed a total of 3 sinks, 2 toilets, 2 potty chairs available for preschool children\342\200\231s use. LPA also observed the school age program to have 2 sinks and 1 toilet available for the school-age children. There is a separate staff restroom equipped with a toilet and a sink located in the adjacent building which is the main office area for staff. The isolation area for children who are ill will be the main office area. The facility currently has a fenced playground area. Fencing is made of a brick wall which is at least four feet high. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. Drinking water is available via children personal water bottles. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. The Department has verified that the facility has obtained a valid fire clearance for the increased capacity the facility is requesting, and that the temporary increase of capacity will not be detrimental to the health and safety of children in care. Page 3: The waiver request is pending final approval from the department. Once approved, the waiver will allow the facility to operate beyond its licensed to serve (40) children: (4) infants ages 0- 2 years in Room #1, (22) preschoolers ages 2- 6 years Room #2 and#3, and (14) school-age children age TK- 12 years in the school-age portable room while the State of Emergency is in effect. The facility license will remain unchanged. The waiver will only be in effect as long as the Department determines is necessary to address the direct impact of the fires identified in the Proclamation and shall automatically expire upon termination of EO N-2-25 and EO N-3-25, whichever occurs first. Once the waiver expires or is terminated, all licensing requirements shall be reinstated immediately, without any further action or notification by CDSS. CDSS may rescind or modify this waiver for any reason including but not limited to new federal, state, or local directives or guidance or for a failure to comply with the terms and conditions of this waiver. Exit interview conducted and report was reviewed with the licensee representative, Turosca Allen. A notice of site visit was given to applicant representative, and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
