What the inspector wrote

On May 30, 2025, Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced case management inspection for the above facility. LPA was greeted by Director, Turosca Allen, who guided LPA on a tour of the facility. The purpose of this inspection is to follow up on a self-reported incident that occurred at the facility on 05/12/2025 . This is a Preschool / School aged Program that operates Monday \342\200\223 Friday from 6:00 am \342\200\223 10:00 pm. There are currently 32 children enrolled and LPA observed 25 children along with 3 staff members. In course of the investigation, LPA interviewed Staff #1 (S1) and Child #1 (C1). LPA did not interview Child 2 (C2), as C2 was napping during the course of the inspection. Brief summary of incident: Child #1 (C1) and Child #2 (C2) were playing and pulling on each end of a toy broom on the play yard. Staff 1 (S1) told both children to stop and C2 let go of the broom, causing C1 to fall on her left arm. S1 observed C1 fall and provided an ice pack. Per S1 there was no bruising or swelling of the left arm. The facility contacted the parents right away and continued to observe C1. C1 continued to have a normal day, but did complain of mild pain on left arm. 2 days later, the parents of C1 called the facility to inform them that C1 had broken left arm. The facility captured the incident on the facility cameras, and LPA was able to observe the incident. Based on the Ouch report, there was no bruising or any visible injury to C1. Page 1 of 2 LPA toured the area on the play yard where the incident occurred. Based on interviews and observations conducted there was no corroborating information to determine that there was a lack of supervision during this incident. The incident was deemed an accident. All reporting requirements were met. There are no deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days during facility\342\200\231s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and a copy of this report was provided to Director, Turosca Allen.
