What the inspector wrote
On 12/02/2025, Licensing Program Analyst (LPA) Kruz Long conducted an announced Case Management licensee-initiated inspection
due to Licensee is requesting capacity increase. Upon arrival, LPA met with Licensee Chen Zuo
who guided LPA on a tour of the facility.
LPA observed 5 children and 2 caregivers. Operating hours are from 8:30am to 6:00pm. Fire Safety Inspection clearance was granted on 11/21/2025.
This is a two story home with: First floor contains a living room, dining room, kitchen, bathroom and attached garage. Second floor contains 5 bedrooms and 3 restrooms.
Main care is provided in the living room, dining area, first floor restroom and rear yard.
Off limit areas are the kitchen and entire second floor. (There is a round water fountain located in the front yard approximately 2 1/2 feet in diameter. Water fountain is empty of water. Per Licensee, children do not have access to the front yard.)
Per Licensee, there are 2 adults and 2 children residing in the home (Licensee, husband, 2 minor children). All adults have criminal record clearance. The home is clean, orderly, comfortable, and well ventilated. Licensee's poisons, detergent, cleaning compounds, medications and other items which could pose a danger to child are stored where they are inaccessible to children. LPA observed working smoke detector and carbon monoxide detector, fully charged fire extinguisher, and working cellular telephones. Fireplace is securely screened. There are several age-appropriate toys and a first aid kit on the premises.
Per the licensee, there are no firearms on the premises. Fireplace in the dining area is securely screened. Barricades between care areas and off limit areas were observed. Licensee has current First Aid and CPR, expires 06/2026. Outdoor play areas are fenced. Cots are provided for napping. Licensee is Mandarin speaking.

, expires on 07/19/2026. All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years.

. The role and responsibilities of being a mandated reporter were reviewed. Applicant reminded that 100% supervision is required at all times to children in care. Applicant was advised on how to access forms and Regulations for Family Child Care online at
www.ccld.ca.gov
. Applicant was made aware that it is her responsibility to know the regulations as well as anyone who assists in providing care. Applicant advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility\342\200\231s phone number; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco in a private residence that is licensed as a family child care home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exersaucers and any other items that fall into that category.
Criminal Record Clearance- FCCH
Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
Licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B (Same day/ASAP or within next business day within normal business hours 8-5 and written report within 7 days). Pamphlet providing Information regarding, Seat Belt Safety and Notification of Parent's Rights poster (Monterey Park Regional Child Care Office) was provided. The "Notification of Parent's Rights" poster must be posted in an area of the home accessible to parents. The information regarding new legislation with regards to exemptions and Parent\342\200\231s Rights was also discussed.

LIcensee was advised that the
Notice of Site Visit
must be posted at the entrance of the facility for a period of
30 days
. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224).
Landlord Consent- FCCH
The Licensee provided proof of control of property. LICENSEE RENTS/LEASES THE HOME AND HAS LANDLORD CONSENT: Because the LIcensee rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
Incidental Medical Services (IMS)- FCCH

LPA reviewed with Licensee the LIC 311D, Forms/Records to Keep in your Family Child Care Home, children\342\200\231s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the Licensee.

Megan\342\200\231s Law- FCCH
Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
MyChildCarePlan.org- FCCH
Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Applicant is ready for licensure.
