What Happened
Informal office meeting regarding capacity increase application. Facility history review reveals serious compliance pattern: 8 Type B deficiencies cited 09/24/25 (all corrected), but critical 10/27/25 incident where inspector observed apparent capacity violation—2 children observed in yard initially but not found inside facility during inspection, suggesting possible hiding or misconduct. Two Type A violations issued for operating over capacity and staffing ratio violations. Department will grant large family child care license but with frequent inspections for one year.

- 10/27/25 critical incident: LPA observed 2 female children and male adult in backyard through fence; upon entering, licensee claimed to have gotten gate remote; only 6 children visible inside, but total of 8 children (6+2 observed outside) present during inspection, exceeding capacity by 2
- Licensee's husband left home during facility operation hours; licensee's claim of only 6 children in care contradicted by initial observation
- Cameras not functioning during inspection

This requirement was not met as evidenced by: Based on LPA Eivazian observation on 10/27/25, LPA observed two girls from the side gate in the backyard that did not see them in the facility upon her arrival inside the facility and licensee stated only she had 6 children present. This poses an immediate health, safety and personal right risk to the children in care.
