What the inspector wrote

On February 26, 2025, Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced case management inspection for the above facility. LPA was greeted by Curriculum Director, Joyce Young, who guided LPA on a tour of the facility. The purpose of this inspection is to follow up on a self-reported incident that occurred at the facility on 01/29/2025 . This is an Infant/Preschool Program that operates Monday \342\200\223 Friday from 7:30 am \342\200\223 5:30 pm. There are currently 62 children enrolled and LPA observed 2 infants with one staff and 46 preschool children with 6 staff members. Brief summary of incident: Child #1 (C1) was trying to climb a bookshelf in classroom 5, when the bookshelf fell on top of the C1. Staff #2 was in the room at time of the incident and went to C1 right away. S2 picked up the bookshelf from on top of C1 and made sure C1 was okay. The facility contacted the parents right away and continued to observe C1. Based on the Ouch report, there was no bruising or any visible injury to C1. The shelf was removed from the classroom. In course of the investigation, LPA interviewed Staff #1 (S1). LPA was unable to interview Staff #2 (S2) is no longer employed with the facility. LPA toured the classroom and confirmed that the shelf was removed. Based on interviews and observations conducted there was no corroborating information to determine that there was a lack of supervision during this incident. The incident was deemed an accident. All reporting requirements were met. There are no deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days during facility\342\200\231s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
