Annual Inspection was conducted in Armenian.
On 12/01/2025, at 9:30 am, Licensing Program Analyst (LPA) Shushanik Safaryan conducted an annual random inspection to the above facility. LPA met with Meri Avagyan , Licensee to whom the reason for the visit was explained. Per Licensee, there are 9 children currently enrolled. There were 6 children and licensee`s assistant present upon arrival. The licensee was observed to be operating within the license capacity limitations.
During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per licensee operation hours are Monday to Sunday, 7:00 a.m. to 11:00 p.m.
During this visit, all areas identified that are accessible for children to use were inspected for safety, comfort, and cleanliness.
All areas identified on the facility sketch were inspected. This is a two story town home, that consists of living room , dining room , kitchen , nook ( day care room ) , bathroom #1 on the first floor and 2 bedrooms and 1 bathroom on the second floor, garage.
Per licensee the children are using bathroom #1 in the hallway next to the laundry room on the first floor , nook next to the kitchen which will be part of the day care on the first floor , dining room , living room , kitchen , backyard( fenced).
Areas inaccessible to the children will be entire second floor, garage. LPA observed stairs to the second floor blocked to prevent children access.

Per licensee, parents will enter the home through the main entrance which leads to the living room and side gate from Doran st that leads to the yard area.
Applicant's husband , Karen Hakobyan is licensed under 542 W Louise st , Glendale , CA 91206, with facility number 198021624.
Licensee, Meri Avagyan requesting to share outdoor play area between day cares on staggered schedule, new updated sketch obtained during this visit.
Per licensee , Meri Avagyan outdoor area for her day care will be used from 9:30 am-10:40 am in the morning and 3 pm-3:50 pm in the afternoon. Hakobyan FCC will use outdoor area between 10:50 am-11:20 am and 4:00 pm-4:50pm.
At 9:30 am ,Licensee guided LPA to the tour of the home. LPA was toured starting from the living room, where 6 children observed with an assistant. LPA observed small tables, chairs , toys. Next LPA toured the dining area and observed all electrical outlets were covered. LPA observed child safety gate blocking stairs to the second floor. LPA observed another gate blocking kitchen. In the kitchen , LPA observed child safety latches on the drawers where sharp items and cleaning supplies are stored. Per licensee , children use bathroom in the hallway. LPA toured the bathroom and observed child safety latch on the cabinet under the sink where cleaning wipes were stored. LPA observed detergents were stored inaccessible to the children. Per licensee, children pass through the nook area to exit to the outdoor area. LPA toured the nook area and observed dining table and chairs. No hazards observed on this day. Children use sliding doors to exit to the outdoor play area.
The outdoor play area was observed to be fenced and gated. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that can pose danger to children. No bodies of water were observed on this day. LPA toured outdoor play area and observed turf area, small play structures , toys. LPA observed garage where children`s toys were stored. Per licensee, she understands garage is off limit and it is not used for day care. LPA observed sliding doors to the Hakobyan FCC unit . Per licensee , no children enrolled in Hakobyan FCC yet and she understands commingling is not allowed.
Facility License, Emergency Disaster Plan, Parent\342\200\231s Rights Poster, and Earthquake Preparedness Checklist were observed to be posted in the living room of the home. A current children\342\200\231s roster was available for review.

There is telephone service via a cellphone that stays at the facility during operation hours. There is central air and heating in the home.
Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Per Licensee, there are no poisons in the home. LPA advised that any poison should be locked under key or combination lock. Licensee states that there are no firearms or weapons stored in the home.
The valve on the required 2A 10BC fire extinguisher indicates fully charged and LPA observed receipt with a purchase date of 11/302025. Smoke and carbon monoxide detectors were observed in the facility.
All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill documented was conducted on 11/25.
The home is observed to be clean and orderly. There are first aid supplies available in the kitchen area. There are age-appropriate toys available for children.
Per licensee, she provides food for children in care. Licensee was advised that if food is brought from the children\342\200\231s homes, all containers must be labeled with child\342\200\231s name and properly stored or refrigerated.
Licensee records were reviewed. The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 10/2026.
LPA observed that the Licensee does have proof of Mandated Reporter AB 1207 compliant Child Care Training Certificate on file with expiration on 12/2026.

Children\342\200\231s records were reviewed, including emergency information, and were observed to be complete. Per licensee , she does not care for infants at this time.

Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.
LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family childcare facility.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
No infants observed in care at this time .LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
LPA advised the licensee how to access forms, regulations and quarterly updates on the Child Care Licensing website at

.
Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee Meri Avagyan , confirmed that there are no Registered Sex Offenders living in the facility.

Currently, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited.
