What the inspector wrote

On Thursday, February 5, 2026, a Case Management - Deficiencies visit was conducted during a Complaint investigation to cite for deficiencies found during the investigation. Upon arrival, Licensing Program Analyst (LPA) Staicy Perry was greeted by Owner, Sheela Deraniyagala who guided LPA Perry on a tour of the facility. Census was taken and LPA Perry observed 35 children in care. Per Owner, Sheela Deraniyagala there are currently 54 children enrolled at the facility. During observations, interviews and record review, it was determined that S#1 and S#2 are not fingerprint cleared, which poses an immediate health, safety or personal rights risk to persons in care. The deficiency listed on the following page was observed by the LPA Perry and is being cited in accordance with California Code of Regulations Title 22. One type A deficiency is being issued today for regulation 1596.871(c)(1)(A) Criminal Record Clearance. Please see attached LIC 809-D for citation. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon their return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled child for the next 12 months. A signed Acknowledgement of Receipt (LIC9224) shall be in each child’s file, acknowledging receipt. The following is being cited in accordance to Title 22 of the California Code of Regulations and Health and Safety Code of the State of California. Civil penalty has been assessed. Exit interview conducted with Owner, Sheela Deraniyagala. Copy of this report provided Owner, Sheela Deraniyagala. Appeal rights were provided and explained. Notice of Site visit posted. Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100 civil penalty.

    (A) Subsequent to initial licensure, a person specified in subdivision (b) who is not exempt from fingerprinting shall obtain either a criminal record clearance or an exemption from disqualification, pursuant to subdivision(f) of this section or Section 1522.7, from the State Department of Social Services prior to employment, residence, or initial presence in the facility.This requirement is not met as based on observation, interview, record review, the licensee did not comply with the section cited above S#1 and S#2 did not have criminal record clearance, which poses an immediate health, safety or personal rights risk to persons in care.
