Correction check·February 13, 2026

Licensing Program Analyst (LPA) Perry conducted a Plan of Correction (POC) visit on February 13, 2026, to verify and clear (one) Type A deficiency previously cited on 2/5/2026. Upon arrival, LPA Perry was greeted by Teacher, Elvia Rivera at the facility. A tour of the facility was conducted, and a census was taken, a total of 41 children in care at the time of the visit. Per Teacher, Elvia Rivera Owner Sheela Deraniyagala is out in the morning due to a doctors appointment. LPA Perry spoke t o S hee la Deraniyagala over the phone during the visit who stated that she will arrive later in afternoon due to a medical appointment. LPA reviewed the Guardian system to verify fingerprint clearance for Staff #1 and Staff #2. Clearance and proper association with the above-named facility were confirmed. The deficiency cited on 2/5/2026, has now been corrected and cleared. A Clearance Letter will be issued to the facility for their records. An exit interview was conducted with Teacher, Elvia Rivera. A copy of this Report and Appeal Rights were provided. Additionally, the Notice of Site Visit was issued and must be posted for a period of 30 days. Failure to post as required will result in a $100 civil penalty.
