What the inspector wrote

On 2/06/2025 at 1:50 pm, Licensing Program Analyst (LPA), Crystal Green conducted an announced Pre-Licensing inspection for an Emergency Relocation due to the recent LA Fire Disaster. The previous facility 198017931 was licensed to operate at 1434 N. Altadena Drive, Pasadena, CA 91107 as a Preschool Program with a capacity of 80 children. Upon arrival, LPA met with Applicant Representative, Carina Hu. The applicant, B\342\200\231 Nai Simcha Jewish Community Preschool, has requested to be licensed to serve 32 children: (8) Infants from ages 0 to 24 months in the Infant Room 1 and Infant Nap Room and (24) Preschoolers from ages 2 to 6 years old in Preschool Rooms 1 and 2, and 3. Additional licensed area will be the Cry Room located on the 1st floor that will be used as an ill isolation room. Hours of operation will be Monday to Friday from 7:30 am to 5:30pm. This facility will share the premises with All Saints Church. The childcare center will be located on the 2nd floor of the church annex building. Parents will access the facility from the church parking lot located from N. Euclid Ave. Per the Applicant Representative, Parents will utilize the loading zone to conduct drop off and pick up. Parents will have the option to use the elevator with an issued key fob or will escort their child within the Main Annex entrance using the stairwell. Staff will be available at both entrances to assist with drop off and pick up. Parents with infants will escort their child directly into the facility. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed applicant representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, applicant representative stated that they will keep it in a locked medicine box located in the Director\342\200\231s office area. A fully equipped first aid kit is also kept in the Director\342\200\231s office area. There is an operational carbon monoxide detector on site located within each classroom. All required licensing documents were observed posted near the main entrance of the facility. Children will be signed in and out at the main entrance of the facility. Page 2: LPA continued to tour the facility and measured all indoor and outdoor activity space. The total indoor activity space for the Infants measured 408.34 square feet which can accommodate the requested infant capacity. The total indoor activity space for Preschoolers measured 1,181.22 square feet which can accommodate the requested preschoolers. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including small tables, chairs, cubbies, 1 crib, napping cots, bookshelves, and other activity supplies for the children. The toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water will be available to children via filter water dispenser machine. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 1/30/2025 by LA County Fire Department. Feeding tables have broad-based legs and seats that are in good repair. Cribs meet regulatory requirements. Crib area was observed located in a separate room with walls which will act as a soundproof barrier with a door. Per the Applicant Representative, they will obtain additional cribs as enrollment for infants increase. Sleep logs will be kept in with the Infant nap classroom. Needs and Services Plans and Sleep Plans will be kept in the Infant Room. Licensing staff observed a total of 5 sinks, 4 toilets available for children\342\200\231s use. Per the Applicant Representative, the preschool children will use the restroom label for boys and the infant will use the restroom labeled for girls. Staff will escort children at all times to the restroom. LPA did not observe a barrier on the 2nd floor to prevent access to the stairwell. There is one changing table located in girl\342\200\231s restroom that was observed for the infants. Changing table have at least 1\342\200\235 padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3\342\200\235 high and the tables was observed located within arm\342\200\231s reach of a sink. There is a separate staff restroom equipped with a toilet and a sink located on the 1st floor near the Cry Room. The isolation area for children who are ill will be located on the first floor in the Cry Room. Page 3: Facility will provide AM and PM snacks and Lunch will be provided by the parents. LPA observed a minifridge located near the sink located within the infant nap room. LPA also observe a kitchen area located next to the cry room located on the first floor. The facility currently has a (2) fully fenced playground area one is designated as the Infant Yard and Preschool Yard. Fencing was observed to be at least four feet high. The total square footage for the designated Infant outdoor activity space is 1,330 square feet, which can accommodate the requested infant capacity. The total square footage for the designated Preschool outdoor activity space is 7,012.50 square feet, which can accommodate the requested preschool capacity. LPA observed the Infant and Preschool yard to have sufficient shade in their designated space. There are wood chips available within the preschool yard and sand available within the infant play area which is sufficient cushioning. The infant yard was observed to have sufficient play equipment. LPA observed a large climbing rope play structure located within the preschool yard to be for ages 5 to 12 years old, which will need to be made inaccessible to the preschool children. There are sufficient outdoor age-appropriate toys, tricycles, and play structure for ages 2 to 5 years old available on the preschool playground. Drinking water is available via portable water jug. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with the applicant and discussed the Child Care Licensing Safe Sleep webpage at: Page 4: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed the applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. The following corrections are needed prior to the issuance of the license: The Applicant must obtain and submit proof of a barrier to prevent access to the stairs leading to the first floor. The Applicant must obtain and submit proof of the play structure located within the preschool playground to made inaccessible. LPA discussed with the applicant representative the additional pending documents that must be submitted to complete the application process. However, at this time due to the emergency order issue by the governor, the applicant can submit within 60 days. Applicant understands that the facility may be issued a provisional license in the event the additional requested documents need time for completion. Once all corrections are received and reviewed, A license will be issues for (32) children: (8) Infants from ages 0 to 24 months and (24) Preschoolers from ages 2 to 6 years will be granted.
