What the inspector wrote

On 3/03/2025 at 10:15 am, Licensing Program Analyst (LPA), Crystal Green conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPA met with Applicant Representatives, Geoff Ramsey- Ray and Jasmine Partida. The applicant, Village Playgarden LLC., has requested to be licensed to serve 48 preschool children from ages 2 to 6 years old in classroom 1, 2, and 3. Hours of operation will be Monday to Friday from 8:30 am to 6:00pm. This facility is located on the premises of Barnhart School. The childcare center will be located near the main office of the school. Parents will escort their child to their child to their designated classroom once their child is signed in. After 9:00 am parents will check in via Barnhart School main office. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed applicant representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, applicant representative stated that they will keep it in a locked medicine box located in the Director\342\200\231s Office. A fully equipped first aid kit will be available in the main office. There is an operational carbon monoxide detector on site located within each classroom. LPA continued to tour the facility and measured all indoor and outdoor activity space. The total indoor activity space for the preschool measured 1891.89 square feet which is sufficient to accommodate the requested capacity of 48 children. Per Mr. Ramsey- Ray, there is a private elementary program that is currently occupying the classrooms and they are expected to vacate the classrooms by March 14th. LPA did not observe the indoor activity space to be complete with age-appropriate furniture and equipment. Page 2: Drinking water will be available to children via filter water jug within each classroom. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 1/05/2025 by Arcadia Fire Prevention Bureau. Licensing staff observed a total of 7 sinks, 3 toilets, 1 urinal available for the preschool children\342\200\231s use which is sufficient to accommodate their requested capacity. LPA observed the restrooms to be labels as Girls and Boys restrooms. Per Mr. Ramsey- Ray, the restrooms will be shared with the Barnhart School TK and Kindergarten program. There is a staff restroom equipped with a toilet and a sink located next to classroom 2. The isolation area for children who are ill will be located in the Nurse room next to the Main Office. Facility will provide AM and PM snacks and Lunch will be the parents. Per Applicant Representative, food prep will be handled within each classroom. The Main Office area and food storage areas were observed to be free of rodents and/or vermin. The facility will have access to a fully fenced playground area that is currently designated for use by the Barnhart School TK and Kindergarten program. Fencing was observed to be at least four feet high. The total square footage for the outdoor activity space is 7,250.00 square feet, which is sufficient for the requested capacity. Per Mr. Ramsey- Ray, a waiver will be requested to allow the preschool program to share use of the outdoor space using a staggered schedule. A playground waiver to share the Infant playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. LPA observed the outdoor area to have insufficient shade via fixed shade structures. There is a climbing structure located within the play area for children from ages 5 \342\200\223 12 years old. Per Mr. Ramsey- Ray, staff will ensure that children under the age of 5 will not be allowed on the climbing structure. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Page 3: Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPA reviewed with applicant representatives the LIC 311A, Records to Be Maintained at The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Page 4: The following corrections are needed prior to the issuance of the license: The Applicant must submit proof of the required licensing documents posted near the entrance to the facility. The Applicant must obtain and submit proof of age-appropriate furniture and play equipment for the preschool classrooms. The Applicant must submit proof of obtaining step stools for the sinks that will be available to the children. The Applicant must obtain and submit proof of napping cots for children. The Applicant must submit a waiver request to allow for the shared use of the outdoor space with the TK/Kindergarten program. The Applicant must submit a waive request to allow for the shared use of the TK/Kindergarten restrooms. The Applicant Representative, Geoff Ramsey- Ray, also had a license for Large Family Childcare Home 198016669 that was destroyed in the Eaton Fire. Per Applicant, this application request is in response to the recent LA Fire Disaster Emergency Relocation. LPA discussed with the applicant representative the additional pending documents that must be submitted to complete the application process. Applicant understands that the facility may be issued a provisional license in the event the additional requested documents need additional time for completion. Once all corrections are received and reviewed a provisional license will be issued for (48) preschool children from ages 2 to 6 years old in classrooms 1, 2, and 3. Exit interview conducted and report was reviewed with the Applicant Representative, Geoff Ramsey- Ray.
