What the inspector wrote

On 03/10/26 at 8:40 a.m., Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection. LPA met with Laura M Cabral, director who guided LPA on a tour of the facility. LPA provided director with a copy of the Entrance Checklist for Child Care Centers form (LIC 125). The preschool program operates Monday \342\200\223 Friday from 8:15 AM to 3:15PM. This facility is located on the premises of Montrose Girls Scouts of Greater Los Angeles. The childcare center is located on the 2nd floor of the building which has direct access to the street side. Per director, there is also a Parent/Child program that operates out of the first floor of the building. The Parent/Child program is also under Pasadena Waldorf School, which operates daily from 9:00 AM to 11:00AM. Per director, Parent/Child program serve children 1 year- 2 years old and 9 months old. Per director, Parent/Child program use the outdoor space on a shared schedule. LPA informed director to submit a waiver request to use the outdoor play area on staggered schedule among Preschoolers and Parent/Child Program by 03/20/26. All areas identified on the Facility Sketch were inspected. The facility has one big classroom, Poppy classroom. Upon arrival, the following staff were present during this inspection; Poppy classroom: 20 children were playing in the classroom with three staff. The facility was observed to be within the license capacity and limitations. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times. The following was observed during the tour of the facility: REPORT CONTINUES ON NEXT PAGE 1 OF 5 PHYSICAL PLANT LPA observed the facility to be clean, safe, sanitary and in good repair. Per director, cleaning crew clean the facility daily after hours. Furniture and equipment was inspected for age appropriateness and good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings in the entrance hallway. Parents take Linens home each week on Friday or as needed and return it on Monday. Napping equipment (cots) were observed to be stored in a storage within the classroom. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in the classroom. Per director, children use their water bottles, cups and water pitcher indoor. Disinfectants, cleaning solutions, medication, and other items that are dangerous to children, were inaccessible to children. Director states there are no poisons stored at the facility. Director tested the carbon monoxide detectors in the classroom. It sounded off the alarms and heard by LPA to be functional. The required 2A10BC fire extinguisher was anchored to the wall in the classroom. It was serviced on 10/17/25 as read by LPA from service tag. LPA observed required posting information were posted on the wall on the parent board in the main entrance hallway. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. LPA observed a child safety gate was installed in the kitchen to make it inaccessible to the children. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal. Outdoor playground equipment is in a safe condition, natural base outdoor playarea, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate natural shade by trees in the play yard. Availability of outdoor drinking water was observed. Per director, children are using the water fountain for drinking which also was tested for Lead. LPA advised that no children shall be left without the supervision of a teacher at any time. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this inspection. Director states there are no weapons or firearms on the premises. REPORT CONTINUES ON NEXT PAGE 2 OF 5 FACILITY RECORDS Facility representative, director, Laura Cabral was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Six Children\342\200\231s Records were reviewed for completeness; Inspection of required forms was made and documented on the LIC 857. Six out of six reviewed children files were completed. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. LPA also reviewed staff records. The review of three Staff records was documented on the LIC 859. All reviewed three staff files were completed. Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present had proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. LPA observed LIC 500, Personal Report and LIC 308, Designated teacher to act as director on parents board. Children's roster was reviewed and is current. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. LPA observed sign in/out sheet was placed inside the entrance hallway. During this inspection 20 children were present, and all 20 children were singed in. Disaster drill log was available, last drill was conducted on 02/25/26. Per director, they do not provide transportation. Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. Preschoolers are provided AM snack and children bring their own lunch and PM snack from home. First Aid supplies were observed in the classroom in a cabinet. According to director, medication is only administered to a child when accompanied with a doctor's note and is stored in the child\342\200\231s classroom. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in the entrance hallway. The ill child will use the staff restroom located in the first level during isolation which is disinfected immediately after use per director. REPORT CONTINUES ON NEXT PAGE 3 OF 5 LPA discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Medication: Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. Facility completed testing on 09/11/25 and no lead exceedance. LPA asked director to submit a copy of labeled facility sketch, LIC 9275 and LIC 9276 by 03/20/26. LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov . REPORT CONTINUES ON NEXT PAGE 4 OF 5 The deficiencies listed on the following pages were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809D for deficiencies that are being cited and need to be cleared to protect the children\342\200\231s health & safety. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today. Facility representative, Laura Cabral was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
