What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 9/15/2025 and was reported to the Department on 9/15/2025. A risk assessment was conducted. LPA met with assistant director Elizabeth Alvarado. Reason for the visit was explained. There were 28 children and 6 staff. On 9/15/2025, an unusual incident report was made to the Department regarding an incident where a preschool child pulled on a vine of a potted plant while lying down on his cot. The potted plant fell down onto his cot, breaking and cutting his hand during naptime. First aid was applied immediately and parents were called. Parents picked up within 20 minutes and took child to ER. Parents informed facility on Friday night that child got 4 stitches on left hand. During todays visit, assistant director stated that child has been recovering well. He stayed home for one week per parents preference and returned with sand box restrictions. Per assistant director Liz, as part of their corrective action, facility has removed all ceramics planters and replanted into plastic pots and moved all planters in all classrooms to be placed lower. LPA observed and inspected each of the classrooms. All classroom pots have been replanted into plastic pots. There is one ceramic pot in the younger preschool class by the teacher sink. Children do not have access to the ceramic pot. LPA did observe a large metal planter on top of a shelf in the toddler room, that can easily be pushed forward by a strong toddler falling onto the other side. Per assistant director, they currently do not have any children in enrolled in the toddler program and can relocate the large metal planter to the floor before toddler children begin care. *continued on next page... Based on information obtained during this investigation, no follow up is necessary regarding the incident reported. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
