Page 1: On 01/29/2026 at 10:15 am, Licensing Program Analysts (LPAs), Crystal Green and Staicy Perry conducted an announced prelicensing inspection for Change of Ownership. LPAs met with Applicant Representatives Nancy Kawai and Jenna Vazquez. This facility is currently licensed as a preschool program with a capacity of 35 children from ages 2 until first grade. The Applicant, Kawai Kids II, LLC, has requested to be licensed for 46 children: (9) infants from ages 0 to 24 months in Bldg. 2 Infant room 1 and crib room, (5) toddlers from ages 18 to 36 months in Bldg. 1 Toddler Rm 4, and (32) preschoolers from ages 2 to 5 years old in Bldg. 1 Preschool Rms 1-3. Hours of operation will be Monday to Friday from 7:00am - 6:00pm. This facility currently operates on the premises of American Hungarian Baptist Church. Present during today\342\200\231s inspection were 16 children with 4 staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Nancy Kawai that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Nancy Kawai stated that they will be stored in a high cabinet within the Kitchen area. A fully equipped first aid kit is available within each classroom. There is an operational carbon monoxide detector on site located throughout the facility. All required licensing documents were observed posted near the entrance of each classroom. Children will be signed in and out using the Brightwheel Application near the front entrance of each classroom. Page 2: LPAs continued to tour the facility and measured all indoor and outdoor activity space. Total infant indoor activity space measured 363.05 square feet, which is sufficient to accommodate the requested capacity of (9) infant children. Total toddler indoor activity space measured 227.59 square feet, which is sufficient to accommodate the requested capacity of (5) toddler children. Total preschool indoor activity space measured 1336.79 square feet, which is sufficient to accommodate the requested capacity of (32) preschool children. LPAs observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, cribs, napping mats for infants who can climb out of a crib, bookshelves, and other activity supplies for the children. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the classrooms via filtered water jug and child\342\200\231s personal drinking cup. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 1/13/2026. Feeding chairs and tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. Feeding chairs are made of washable, moisture-resistant material. Changing tables have at least 1\342\200\235 padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3\342\200\235 high and the changing table is within arm\342\200\231s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via located in a separate room adjacent to the Infant activity room. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs will be kept in the crib area. Needs and Services Plans and Sleep Plans are kept in the infant room. LPAs observed a total of (4) sinks, (2) urinal, (2) toilets, and 1 changing table within arm\342\200\231s reach of sink available for preschool children\342\200\231s use. These are sufficient to accommodate the requested capacity of preschool children. LPAs observed a total of (2) sinks, (1) toilet, and a changing table located within arm\342\200\231s reach of a sink available for infant children\342\200\231s use. LPAs did not observe a separate restroom available for toddler children. Per Applicant Representative, children enrolled in the toddler program will not be potty trained. LPAs informed applicant representatives that a change table must be available for toddler children and located within arm\342\200\231s reach of a sink. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the Director\342\200\231s office and ill children will use the staff restroom. Page 3: Facility will provide Breakfast, AM/ PM snacks and Lunch. The kitchen area currently includes (2) refrigerators, commercial stove and sink, and food prep counter. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via locked doors. The facility currently has (2) fully fenced playground areas. Fencing was observed to be made of chain links and concrete walls which are at least four feet high. The total square footage for the infant outdoor activity space is 845.58 square feet, which is sufficient to accommodate the requested infant capacity. The total square footage for the toddler outdoor activity space is 800 square feet, which is sufficient to accommodate the requested toddler capacity. The total square footage for the preschool outdoor activity space is 2901.16 square feet, which is sufficient to accommodate the requested preschool capacity. The toddler outdoor activity space was observed to not have physical barrier surrounding their designated area and were missing age- appropriate toys and play equipment, shade, and seating area. Shade was observed is provided via fixed shade structure and shade trees in the Infant and Preschool yards. There are sufficient outdoor age-appropriate toys and play equipment available on the Infant and Preschool playground. There is a climbing structure on the preschool playground for children ages 2- to 5 year old which is properly anchored. There is adequate cushioning in fall zones of climbers provided by cushioning. Drinking water is available via Filter water jug. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Page 4: Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPAs discussed with applicant representative the LIC 311A, Records to Be Maintained at The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. LPA discussed the safe sleep regulations with applicant representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed applicant representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. Page 5: The Applicant Representative, Nancy Kawai, also is an existing licensee for Large Family Child Care Home 198013467. During this inspection, Nancy Kawai has been advised that in order for her to maintain her family childcare license, she must be present in the home 80% of the time. The following correction is needed prior to the issuance of the license: The Applicant must obtain a fencing barrier, age- appropriate toys and play equipment, shade and seating area. The Applicant must obtain a changing table within arms reach of a sink in the toddler classroom. The Applicant must submit an updated LIC 200A. Applicant Representative understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. A license for (46) children: (9) infants from ages 0 to 24 months in Bldg. 2 Infant room 1 and crib room, (5) toddlers from ages 18 to 36 months in Bldg. 1 Toddler Rm 4, and (32) preschoolers from ages 2 to 5 years old in Bldg. 1 Preschool Rm 1-3. will be granted. Exit interview conducted and report was reviewed with the Applicant Representative, Nancy Kawai.
