What the inspector wrote

On 05/06/2026 Licensing Program Analyst (LPA) Mary Silva conducted an unannounced POC (Plan of Correction) inspection to ensure that the Type A deficiencies cited on 04/17/2026 have been cleared. LPA met with licensee Sara Nava who guided LPA on a tour of the facility. There were no children present during this inspection. Per licensee knowledge there are 6 children enrolled. Inspection was conducted in spanish. LPA reviewed a total of 6 staff files including file for licensee, at this time facility does not have qualified staff to provide care and supervision to children in care. Reviewed requirements needed for staff and licensee. Licensee was reminded in order to operate the facility there must be qualified staff to meet the needs of children. LPA advised the licensee how to access forms, regulations and quarterly updates on the Child Care Licensing Website at: www.ccld.ca.gov Based on the information verified during the inspection of the incident, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety. LPA informed Licensee Sara Nava that this report dated 05/06/2026 documents 1 type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety and personal rights of the children in care. LPA informed Licensee, Sara Nava to provide a copy of this licensing report dated 05/06/2026 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parent/guardian for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement must be placed in the child’s file for verification. Page 1 of 2 Licensee shall post report documenting Type A citation along with Notice of Site Visit Form in an area accessible for review for 30 days or a civil penalty of $100 will be assessed. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Appeal rights provided. Exit interview conducted and report was reviewed with licensee Sara Nava. Page 2 of 2
