What the inspector wrote

On 08/11/2025 at 1:30 pm, Licensing Program Analyst (LPA), Crystal Green conducted an announced prelicensing inspection to add a new toddler program to an existing preschool license which will be consolidated into a new Single License. LPA met with Applicant Representative, Wendy Veas. This facility is currently licensed as a preschool program (191502175) with a capacity of 74 children serving ages 2 until entry into first grade in (7) classrooms: Dragonflies, Caterpillars, Butterflies, Lady Bugs, Ants, Rainbows, and Busy Bees. The Applicant, PlayFactory Preschool, has requested to decrease their preschool program capacity and add a new toddler program the overall capacity will remain at 74 children: (13) toddlers from ages 18 to 36 months in the Caterpillar classroom and (61) preschoolers from ages 2 to 6 years old in (6) classrooms: Dragonflies, Butterflies, Lady Bugs, Ants, Rainbows, and Busy Bees. The hours of operation will be Monday through Friday from 7:00 a.m. to 6:00 p.m. Visitors and parents will access the facility from the main parking lot from Golden West St. or from the front side located on Garibaldi Ave. Sign in and sign out near the front side entrance and parents will escort their child to their classroom. Present during today\342\200\231s inspection were 34 children with 9 staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Licensee Representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. Page 2: LPA continued to tour the facility and measured the indoor activity space for the new toddler program. The total indoor activity space for the Toddler program measured 426.40 square footage, which can only accommodate the capacity of 12 toddlers. The total indoor activity space for the Preschool program measured 3,081.20 square footage, which is sufficient to accommodate the requested capacity of preschoolers. The Fellowship Hall was inspected today to be used as an assembly area however no measurements were obtained. LPA observed all indoor activity spaces to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available via filter water jugs, and the child\342\200\231s personal water bottle. LPA observed all hazardous items to be inaccessible to children. LPA observed a total of 5 sinks and 5 toilets available for preschool children\342\200\231s use. The proposed toddler restroom will be located adjacent to their classroom and was observed to have 1 sink and 1 toilet and a changing table located within arm\342\200\231s reach of a sink. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the main office. Facility provides Breakfast, Lunch, and AM/ PM snacks. There is a kitchen located on the premises that will provide food for the children. The kitchen was observed to contain a refrigerator, commercial stove, sink, microwave, and food prep counter. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in the kitchen are inaccessible to children via locked cabinet. The facility currently has several fully fenced playground areas. LPA observed the Toddler program to have its own designated outdoor yard. Fencing for the toddler yard is made of vinyl, which was observed to be at least four feet high. The Toddler yard measured a total of 1110.73 square footage, which can accommodate the requested toddler capacity. Shade is provided via fixed canopies. The Preschool yard measured a total of 11,406.41 square footage, which can accommodate the requested preschool capacity. There are sufficient outdoor toys and play equipment available on both playgrounds, which are properly anchored. Drinking water is available via child\342\200\231s personal bottled water and filter water jug. Page 3: LPA observed all hazardous items on the playground to be inaccessible to children. The applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. No corrections are needed. Due to the measurements obtained, A license for 74 children: (12) toddlers from ages 18 to 36 months and (62) preschoolers from age 2 to 6 years old will be granted upon a final file review. Exit interview conducted and report was reviewed with the Licensee Representatives, Wendy Veas.
