What the inspector wrote

On 9/09/2025 at 1:10 pm, Licensing Program Analysts (LPAs), Crystal Green and Shushanik Safaryan conducted an announced Pre-Licensing inspection for a Change of Location. Upon arrival, LPA met with Applicant Representative, Talin Elmedjian. The Applicant, The Busy Mind, LLC., has requested to relocate the preschool program children from The Busy Mind (198021465) located at 3847 Foothill Blvd., La Crescenta, CA 91214. The Applicant is requesting to be licensed for 45 preschoolers ages 2 to 5 years old in classrooms 1, 2, and 3. This will operate Monday to Friday from 8:30am \342\200\223 4:00pm. All indoor and outdoor activity space utilized for the children was inspected today. This location has a 2nd floor, which includes the Director office, staff restroom, and additional storage room. LPA informed applicant representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. A fully equipped first aid kit is located in within each classroom. There is an operational carbon monoxide detector on site located throughout the facility. All required licensing documents were observed posted outside of classroom 3. Children will be signed in and out at the entrance of classroom 3. LPA continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured 1,399.86 square footage, which can only accommodate 40 children. LPA observed all indoor activity spaces to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water will be available in the classrooms via water bottles. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 09/04/2025 by the Los Angeles County Fire Department. Page 2: LPA observed a total of 3 sinks and 3 toilets available for children\342\200\231s use. These are sufficient to accommodate the requested capacity of 45 children. There is a separate staff restroom equipped with a toilet and a sink located on the 2nd floor. The isolation area for children who are ill will be the located on the 2nd floor next to the Director office. Parents will provide their child\342\200\231s lunch. The Facility will provide AM and PM snacks. There is a kitchen on site which contains a sink, prep counter, and refrigerator. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the director\342\200\231s office. Children who are ill will use the staff restroom located on the 2nd floor. The facility has a fully fenced playground area. Fencing is made of concrete brick wall which was observed to be at least four feet high. LPA obtained measurements of the outdoor yard which calculated to be 2,892.16 square footage, which can only accommodate the capacity of 38 children at one time. Per Applicant Representative, a waiver will be requested to obtain the capacity of 40 children. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. Shade is provided via shade tree and fixed shade structure. There are sufficient outdoor age-appropriate toys and play equipment available on the main playground area. Drinking water is available via bottled water. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Per Applicant Representative, food will not be prepared on site, and the facility does not plan to provide snacks that require rinsing until lead testing has been completed. Page 3: Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. Applicant Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Page 4: During this inspection, Licensing staff obtained pictures of the outdoor play area to include with the applicant\342\200\231s outdoor waiver request to obtain the requested capacity. The Applicant Representative, Talin Elemedjian, was informed that the waiver request will be reviewed by the Regional Office for final approval. No corrections are needed. Based on the measurements obtained during this inspection, a license will be issued for (40) children from ages 2 to 5 years old in classrooms 1, 2, and 3 will be granted upon a final file review and final approval of the outdoor waiver request. Exit interview conducted and report was reviewed with the Applicant Representative, Talin Elemedjian.
