What the inspector wrote

On 09/11/2025 at 1:30 pm, Licensing Program Analysts (LPAs), Crystal Green and Elka Chavez conducted an announced prelicensing inspection to add a new toddler program to an existing preschool license which will be consolidated into a new Single License. LPAs met with Applicant Representatives Katrine Ebrahimian and Celine Mirzaians. This facility is currently licensed as a preschool program (198017987) with a capacity of 221 children from age 2 until first grade in (10) classrooms out of Buildings 1, 2, and 3. The Applicant, Preschool Educational Corporation, has requested to change their licensed capacity by adding a new toddler program in building 3 classroom 4 to their existing licensed areas. The Applicant has requested to serve 245 children: (221) preschoolers from age 2 to 5 years old in Buildings 1, classrooms 1- 3, Buildings 2, classrooms 1- 4 and Building 3, classrooms 1- 3; and (24) toddlers from ages 18 to 36 months in Building 3 classroom 4. Hours of operation will be Monday to Friday from 7:30am - 5:00pm. This facility is currently operating on the premises of Living Stone Church. Present during today\342\200\231s inspection were 218 children with 37 staff. Appropriate ratios and supervision were observed. LPA informed applicant representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Applicant representatives stated that they will be located in the main office. A fully equipped first aid kit is located within classroom 2 of each building. There is an operational carbon monoxide detector on site located throughout the site. All required licensing documents were observed posted near the entrance of each classroom. Sign in and out takes place near the entrance of each classroom. Page 2: LPAs continued to tour the facility and measured new indoor activity space. Total indoor activity space for the new toddler program measured 849.72 square footage, which is sufficient for the requested toddler capacity of 24 children. Total indoor activity space for the preschool program measured 9,859.50 square footage, which is sufficient to accommodate the requested preschool capacity of 221 children. LPA observed all indoor activity spaces to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via filter water dispenser. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 09/02/2025. LPAs observed the proposed toddler program to have access to 2 sinks and 2 toilets which is sufficient for the requested capacity of 24 toddler children. A changing table which was observed located within arm\342\200\231s reach of a sink. The preschool program has access to a total of 27 sinks, 20 toilets, and 1 urinal. These are sufficient to accommodate the requested preschool capacity of 221 children. There are (2) separate staff restroom equipped with a toilet and a sink located in Building 1 and 2. Staff assigned to building 3 will have access to toilet and sink located within the Girls bathroom. The isolation area for children who are ill will be the main office and ill children will use the staff restroom located in Building 1 or 2. Facility will provide Breakfast, AM snacks, and Lunch. The facility has a commercial kitchen area which currently includa commercial stove, refrigerator, microwave, and prep food counter. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in the kitchen are inaccessible to children via locked doors. The facility currently has several fully fenced playground area named building 1 outdoor play area, building 2 outdoor play area, building 3 outdoor play areas including the Basketball courts, and the Field area located between building 1 and 2. Fencing is made of wrought iron and chain link around the field which was observed to be at least four feet high. The preschool outdoor areas were observed to be sufficient to accommodate preschool capacity. The total square footage for the designated outdoor toddler space is 1,962.69 square footage, which can accommodate the requested toddler capacity. Page 3: Shade was observed provided via shade canopies. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. Drinking water is available via filtered water jug. LPA observed all hazardous items on the playground to be inaccessible to children. The Applicant Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Licensing staff observe the facility to have completed their lead testing on 02/14/2023. The results were posted on the parent board. Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Page 4: LPA discussed the safe sleep regulations with applicant representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. No corrections are needed. A license for (245) children: (24) toddler children from ages 18 to 36 months and (221) preschoolers from ages 2 to 5 years old will be granted upon a final file review. Exit interview conducted and report was reviewed with the Applicant Representatives, Katrine Ebrahimian and Celine Mirzaians.
