What the inspector wrote

On 05/08/2026 at 12:30PM Licensing Program Analyst (LPA) Mariah Aguirre conducted an unannounced Case Management inspection to the above facility. Upon entering the facility, LPA MAguirre was greeted by Lead Teacher Esther Villa to whom the reason for the visit was explained. Also present at the time of inspection was teacher Sonia Holguian. Lead Teacher Esther Villa lead LPA on a tour of the facility. A census was taken. LPA MAguirre observed 5 children in care with two staff members. The purpose of the inspection was to follow up on an incident that occurred on 04/30/2026 and was reported to the department on 04/30/2026. The incident was reported in a timely manner. Summary of the Incident: P1 informed S1 of an injury that occurred to C1 at the facility. P1 stated C1 disclosed the injury was done by S1. P1 requested further clarification as the incident report done by the facility did not contain detailed information regarding the incident that took place. During today's visit, LPA MAguirre obtained documents pertaining to the incident including but not limited to incident reports, an updated children's roster, an updated personnel roster, and conducted staff and children file reviews. At this time, the incident requires further investigation. A Notice of Site Visit was given to the Lead Teacher Esther VIlla and must remain posted for 30 days. Failure to comply with posting requirements will result in an immediate civil penalty of $100. Exit interview was conducted and a copy of this report was provided to the Lead Teacher, Esther Villa.
