What the inspector wrote

On 05/19/2026 at 10:30 am, Licensing Program Analysts (LPAs) Crystal Green and Veronica Martinez- Gaza conducted an announced Proof of Correction (POC) inspection. Licensing staff met with the Applicant Representative, Chang Mu. This follow-up inspection was conducted to verify the corrective actions required from the prelicensing inspection completed on March 19, 2026. The following corrections were observed: The proposed infant outdoor space was observed to have a physical barrier installed. The proposed infant outdoor space was observed to have age-appropriate outdoor play equipment. The proposed infant indoor classroom #4 was observed to be completed with age-appropriate furniture and equipment. The proposed infant classroom #5 was observed to have a change table located within arm’s reach of a sink. The proposed infant classroom #5 provides a comfortable room temperature LPAs inspected Classroom #5 and observed that the designated crib area was not fully enclosed by a soundproof barrier at least four feet in height. LPAs observed one soundproof barrier panel measuring approximately nine feet in length and four feet in height; however, the barrier did not completely surround the five cribs located within Classroom #5. According to the Applicant Representatives, two additional panels will be obtained to fully enclose the designated infant crib area. During this inspection, Licensing staff also re-measured the proposed infant areas. The total indoor infant activity area measured 764.76 square feet, which is sufficient to accommodate the requested capacity of 20 children. The total preschool activity area measured 4,163.55 square feet, which can only accommodate 118 children. The measurements obtained for the outdoor activity space for the infant yard measured 977.67 square feet, which can only accommodate (13) children at one time, insufficient for the number of children being requested. The preschool outdoor activity space measured 7,142.26 square feet, which can only accommodate (95) children at one time, insufficient for the number of children being requested. The following correction is needed prior to the issuance of the license: The Applicant must submit an updated LIC 200A- Childcare Center Application- to reflect the change in programs served and requested capacity. The Applicant must obtain and submit proof of additional soundproof barrier panels to fully surround the designated crib area located within Classroom #5. Based on the measurements obtained from both indoor and outdoor activity space, the facility can only accommodate 108 children. Once the above corrections are received and approved, A license for (108) children: (13) infants from ages 0 to 24 months in classrooms 4 and 5; and (95) preschoolers from age 2 to 6 years in classrooms 1, 2, 3, 6, and 7 will be granted. Exit interview conducted and report was reviewed with the Applicant Representative, Chang Mu and Kendrick Cheng.
