What the inspector wrote

On 02/06/2026 Licensing Program Analysts (LPAs), Elka Chavez and Mariah Aguirre conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPAs met with Applicant Representatives, Monica Jarvis and Cynthia Guerra. Also present, Debbie Yin, Site Supervisor. Applicant, Hacienda La Puente Unified School District is requesting to be licensed for (24) preschoolers 2 to 5 years old in the State Preschool classroom. Hours of operation will be Monday through Friday from 8:00 am to 3:30 pm the morning session will be from 8:15 am to 11:15 am and afternoon session will be from 12:30 pm to 3:30 pm. This facility is located on the premises of Hacienda La Puente Unified School District Wedgeworth Elementary School. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Cynthia Guerra that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Debbie Yin stated that they will be kept in a cabinet above the sink in the classroom. LPAs observed that the cabinet is kept locked. A fully equipped first aid kit is located in the classroom. There is an operational carbon monoxide detector on site located in the classroom. LPAs did not observe all required licensing documents to be posted in a prominent, publicly accessible area at the facility. Children will be signed in and out at the entrance gate of the outdoor activity area designated to the Kindergarten and State Preschool program. LPAs continued to tour the facility and measured all indoor and outdoor activity space. LPAs observed the State Preschool classroom is located in building C. Total indoor activity space measured 961.63 square feet, Page 1 of 4 which is sufficient to accommodate the requested capacity of (24) Children. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, bookshelves, and other activity supplies for the children. Drinking water is available in the classroom via Brita water pitcher filled with water from the sink and disposable cups. LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 10/25/2025. LPAs observed a total of (2) sinks and (3) toilets available for children\342\200\231s use. These are sufficient to accommodate the requested capacity of (24) children. The isolation area for children who are ill will be the Collaboration Room which is located within the classroom. There is a separate staff restroom equipped with a toilet and a sink. The ill children will use the big stall in the children's restroom. Facility will provide AM/PM snacks and lunch. LPAs observed meals are prepared in the school\342\200\231s cafeteria. LPAs were informed by Debbie Yin that meals are delivered to the State Preschool classroom in a warmer. LPAs observed the cafeteria includes 2 ovens, 3 food warmers, 3 refrigerators, microwave, food prep counter and 2 industrial sinks. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. The facility currently has a fully fenced playground area. Fencing is wrought iron and is at least four feet high. The outdoor activity space will be shared with one of the Kindergarten classrooms. The total square footage for all the outdoor activity space is 9,980.66 square feet, which is sufficient to accommodate the requested capacity. Shade is provided via fixed canopies. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure on the playground for children which is properly anchored. There is adequate cushioning in fall zones of climbers provided by rubber surfacing. LPAs did not observe a manufacturer sticker or manual. LPAs observed parts of the structure were not appropriate for younger children. The structure contains two sections with two slides. LPAs observed the slide area which contains the spiral slide measured approximately 6 feet from the platform to the ground. LPAs recommended additional supervision of younger children while on the climbing structure. Drinking water is available via Brita water pitcher and disposable cups. LPAs observed all hazardous items on the playground to be inaccessible to children. Applicant Representative, Cynthia Guerra, was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Page 2 of 4 For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). If the facility has had a lead test completed within the last five years from the pre-licensing visit date 02/06/2026 and is using the same outlets, you may submit a waiver request from the lead testing requirement to the Regional Office Facility Representative, Monica Jarvis was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPAs reviewed with Applicant Representative, Cynthia Guerra the LIC 311A, Records to be Maintained at the Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Entrance Checklist was provided to the applicant. Facility Representative, Monica Jarvis was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Page 3 of 4 Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. The following corrections are needed prior to the issuance of the license: \342\200\242 The Applicant will need to submit a waiver for shared use of the outdoor space. \342\200\242 The Applicant must submit proof of licensing required documents being posted. Facility Representative, Monica Jarvis understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. Once the final approval is granted for the requested waiver, A license for (24) Preschoolers from ages 2 to 5 years old in the State Preschool classroom will be granted.
