What the inspector wrote

Licensing Program Analyst (LPA) Reiko Jones-Modeste conducted a CASE MANAGEMENT_POC inspection to address several action items discussed during PRELICENSING inspection conducted on April 30, 2021 via TELE_INSPECTION using FACETIME due to SOE. LPA observed the Applicant Grace Lasisi present at the facility. During this tele-inspection the Applicant guided the LPA on a tour of the facility to inspect the following action items: 1. Barricade around playground in parking lot 2. Appropriate ground cushioning and shade per regulations 3. Isolation area Signage 4. Off-limits areas made inaccessible(adjacent staff bathroom, office, side yard and isolation) 5. Required postings (COVID, LIC 311A and Personal Rights, Emergency Disaster Plan, Menus, Parents Rights) 6. Appropriate children\342\200\231s toys and play equipment set-up 7. First Aid kit made inaccessible (missing no-touch thermometer) 8. Carbon Monoxide, Smoke Detectors and Fire Extinguisher 9. Sign in/out sheets 10.Water available in outdoor play yard LPA inspected the areas discussed for proof of correction including parking lot playground with small, white gate used as barricade to the play area as well as ground cushioning and play equipment. LPA also observed a shaded area without barricade and no shade above playground. LPA observed small bucket of water used for washing. LPA advised Applicant to provide drinking water in the outdoor playground. LPA observed side yard inaccessible with stationary wooden gate. LPA LPA observed COVID intake with required COVID posters only. LPA observed off-limits staff bathroom accessible to children in care. LPA observed adjacent off-limits office inaccessible with key lock. COVID intake missing Emergency Disaster Plan, Parents Rights, sanitizer, gloves, masks, menus and no-touch thermometer. COVID inspection pending. Fully charged fire extinguisher observed near playground. Smoke detectors tested and operable in both Classroom #1 and Classroom #2. LPA did not observe Disaster Drill log or a Carbon Monoxide detector, LPA observed appropriate toys, games, children\342\200\231s equipment, tables and chairs in both preschool classrooms(1&2). LPA observed food stuffs for children near cubbies in Classroom #1. Applicant stated food will be provided by parents and stored in the kitchen. LPA observed COVID isolation without signage in an adjacent room inaccessible to children in care. LPA also observed first aid kit inaccessible with no-touch thermometer in Classroom #2 adjacent to playground. LPA advised the applicant how to access forms, regulations and quarterly updates on the Child Care Licensing Website at: www.ccld.ca.gov The following corrections must be completed prior to LICENSURE and are due by: May 27, 2021. 1. Playground without drinking water or shade 2. COVID PPE(gloves, masks, sanitizer, sign-in, thermometer) 3. Emergency Disaster Plan, Parents Rights, Menus, Disaster Drill log posted outside 4. COVID postings in staff bathroom and Isolation area signage 5. Carbon Monoxide detector not available Exit interview was conducted with the APPLICANT during which Appeal Rights were explained and provided. Upon receipt the APPLICANT shall post the Notice of Site Visit. This report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. A copy of this report and all other licensing reports must be made available to the public for 3 years.
