What the inspector wrote

On 10/14/25 Licensing Program Analysts (LPAs) Portia Bowden and Javier Duran conducted an unannounced Required Annual Inspection at the above facility. At 9:30AM LPAs met with Pastor Lasisi who stated Director Alake Lasisi was out of the facility. At 9:45AM Pastor Lasisi called director, LPAs explained the reason for visit and continued guided tour with Pastor Lasisi and Director via telephone. Per Director the facility operating hours are 6 AM to 6PM Monday to Friday. LPA observed all required documents on the facility licensing board (PUB 269, PUB 393, LIC 610, LIC 9224, LIC 613A) LPA did not observe an Emergency Drill log last dated as there are no children currently enrolled. This is a preschool program consisting of 1 classroom serving ages 2-6 and 1 enclosed outdoor play area (parking lot) Per Director there are 0 Enrolled. Facility also houses on the premises a community church. Capacity and limitations as specified on the license are being maintained. Per Director facility does not administer Incidental Medical Services. The following was observed during the tour of the facility: furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. At 10:00AM LPA observed a combination carbon monoxide detector/ Smoke detector on the classroom ceiling. LPA observed mats for sleeping. Per Director cot linens for sleeping will be sent home with children on Friday to be returned to the facility Monday. LPA observed 6 age appropriate toilets, 4 age appropriate sinks in good repair in the bathrooms located in the church. Per Director facility will assist with potty training. Per PIN 21-21-CCP lead testing to be conducted for all equipment used for drinking consumption. LPAs (did not observe Lead testing results. A technical violation will be issued due to no children being enrolled. LPAs will provide consultation to Director regarding Required Lead testing. Per director facility does not use building water. Children in care will be required to bring bottled water. In the event child forgets water, facility has bottled water stored on site. LPA observed Disinfectants stored in a locked cabinet in the staff bathroom, inaccessible to children in care. Per Director there are no poisons on the premises. Per Director facility provides no meals to children in care. Children are required to bring all meals, outside food is not permitted in the facility. LPAs observed no kitchen on site. Outdoor play area was observed to be enclosed with playground equipment is in a safe condition, free of sharp, loose or pointed parts. Outdoor area is located in the parking lot of the church. At 10:30AM LPAs observed multiple cars in the parking lot, per Pastor Lisisi upon enrolling children cars will no longer be parked in the enclosed area. LPA did not review staff files as Director stated they are in her vehicle out of the facility. LPA observed Director\342\200\231s Mandated Reporter certificate exp 8/26 and Pediatric CPR/First Aid Exp 10/25. Per Director she will retake CPR course this month. LPAs did not review Children\342\200\231s files as there are no children enrolled. The name of the child care center director or fully-qualified teacher(s) designated to act in the director\342\200\231s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Two Technical Violations for required lead testing and staff files. Exit interview conducted and report was reviewed with Pastor Lisisi, A notice of site visit was given and must remain posted for 30 days
