What the inspector wrote

REPORT NOT AMENDED---Licensing Program Analysts (LPAs) Alicia Mooberry and Andrea Carter conducted an unannounced complaint inspection to the above facility. LPAs arrived at the facility at 3:00pm and met with Facility Representative Olivia Gibson. LPAs explained the purpose of visit and toured the facility. There is a preschool program under License # 198400158 and an Infant Center License # 198400156 also on the premises. Licensee, Teliscia Martin arrived during inspection. LPA observed 3 children in care. During this inspection, LPA's observed 1 school aged child (age 5) taken to the Preschool program due to there was no staff in the School age classroom. During this investiagation, LPA Mooberry conducted interviews, recorded observations and reviewed files. Based on record reviews and observations, there is a preponderance of evidence to substantiate the above allegation. Exit Interview was conducted with Licensee Teliscia Martin, Appeal rights were discussed and provided.
