What the inspector wrote

REPORT NOT AMENDED Licensing Program Analysts (LPAs) Alicia Mooberry and Andrea Carter conducted an unannounced complaint inspection to the above facility. LPAs arrived at the facility and met with Facility Representative Olivia Gibson. LPAs explained the purpose of visit and toured the facility. Licensee, Teliscia Martin arrived during inspection. LPA observed 8 children in care. During this inspection, LPA's observed 1 school aged child (age 5) taken to the Preschool program due to there was no staff in the School age classroom. During this investigation, LPA Mooberry conducted interviews, recorded observations and reviewed files. Based on record reviews and observations, there is a preponderance of evidence to substantiate the above allegation. The following deficiencies are cited in accordance with Title 22 regulations, see 9099D. Exit Interview was condcuted with licensee Teliscia Martin, Appeal rights were discussed and a copy provided.
