What Happened

Case Management visit found critical violations: facility operating over licensed capacity (17 children vs 14 licensed), staff without fingerprint clearance left alone with 12 children at park, and licensee failed to verify staff clearance before employment. Three Type A deficiencies and one civil penalty (LIC421IM) issued.

Observations

  • Facility census 17 children when licensed for maximum 14 - over capacity by 3 children
  • Staff #1 hired 12/2/24 left facility with 12 after-school children to park without verified fingerprint clearance
  • Staff #1 clearance verification showed staff was NOT cleared despite licensee believing they were cleared
  • Licensee could not provide clearance documentation for Staff #1
  • 5 infants observed in backyard with 2 staff assistants
  • 12 after-school children taken to park by unclearred Staff #1 before Staff #4 arrived to supervise
