On May 20, 2025, at 9:58a.m., Licensing Program Analysts (LPAs) LPA Keneisha Dunlap and Peter Bishop arrived at the facility for an unannounced case management visit. LPA Dunlap announced the purpose of the visit and was granted entry into the facility by Facility Representative- Tiffany Fields. There are 8 children enrolled, and 7 children present, and 2 staff present at the time of inspection. This is a preschool program, and the center hours of operation are Monday- Friday 7:00am-6:00pm. On May 15, 2025, the facility was cited for the following deficiencies: Physical Plant - Technical Violation: 101239(m)(1) - Not enough toys in outdoor area. This deficiency has been cleared as off May 20, 2025. LPA Dunlap and Bishop observed multiple toys on playground area. Physical Plant - Type B: 101238(a) - The playground area has multiple orb webs and sheet webs across the top of the fence area. This deficiency has been cleared as off May 20, 2025. LPA Dunlap and Bishop observed the fence on the playground to be free of any webs. Physical Plant - Type B: 101438.1(c)(4)(A) - 3 out of 3 rugs have stains/soiled. This deficiency has been cleared as off May 20, 2025. LPA Dunlap and Bishop observed 3 rugs have been cleaned. Physical Plant - Type B: 101238.2(b)(1) - The infant playground does not have a shaded area for children to rest. This deficiency has been cleared as off May 20, 2025. LPA Dunlap and Bishop observed the playground to have a canopy. Staff Records - Type B: 101216(f) - 1 out 2 staff had expired/missing CPR & First Aid certificate. This deficiency has been cleared as off May 20, 2025. LPA Dunlap reviewed staff file with expiration dates of Page 1 of 2 Staff #1 5/15/27 On May 15, 2025, the facility was cited for the following deficiencies: Staff Records - Type B: 101216(g)(1) - 2 out 2 staff did not have their Health Screening Report (LIC503) missing from staff files. As of May 20, 2025, this deficiency has not been cleared. Type B violation will be re- issued during today\342\200\231s visit. A notice of site visit was given and must remain posted for 30 days.
