What the inspector wrote
Licensing Program Analysts (LPA) Alicia Mooberry conducted an unannounced Required - 3 Year Inspection on this date. LPA arrived at 1:15pm and met with Diana Ordaz, Licensee, and explained the purpose of the visit. LPA toured the home and observed six (6) children in care (including 2 infants). LPA provided the inspection Entrance Checklist, LIC 126 to licensee. Also present was Consuelo Rodarte, Assistant. The licensee is within the capacity limitations.
Per licensee the hours of operation are Monday-Friday 7:00am-6:30pm. At 4:00pm arrived at the facility, bringing the facility into capacity. All adults present have obtained the required background clearance. Individuals residing in the home were discussed and noted.
This facility is a single family home that consists of two (2) bedrooms, one (1) bathroom, living room, dining room, den, kitchen, shaded and enclosed patio, and attached garage.
The home was inspected for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children.
Areas used by children include: Living room, dining room, kitchen, den, bathroom, and shaded patio and enclosed yard.
Areas off limits include: Bedroom 1 and bedroom 2, attached garage.
The home was observed to be clean and orderly and has central heating and air conditioning. There are outlet covers throughout the home. LPA observed knives and sharp objects in kitchen drawer with safety latch inaccessible to children in care. Cleaning products are stored in locked cabinet under kitchen sink as well as bathroom sink. Licensee was reminded to regularly check the safety locks to make sure they are working properly.

The required (2A10BC) fire extinguisher is located mounted in the kitchen. Smoke detectors and carbon monoxide detector was observed, tested and found to be operable.
LPA observed required posted documentation which included: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form in the den where children in dropped off and picked up.
Toys and other age appropriate material available for children.
Licensee states that there are no poisons stored in the home and understands that all poisons must be lock, not only inaccessible to children. Licensee there are no firearms or weapons stored in the home. Per licensee, here are no pets in the home.
Per licensee, children that become ill during care will be separated form the other children and wait in the living room, away from the other children.
The bathroom that children use is located in the observed to be clean and free of hazards.
LPA reviewed the Safe sleep regulations PIN 20-24-CCP. including LIC 9227 Infant Sleep Plan for infants under 12 months, and Infant sleep log documenting 15-minute sleep checks for infants 0-24 months.
Currently, children are using the enclosed side yard and shaded patio for outdoor play. The outdoor play area was observed to be fenced with toys and other materials for children to play with. LPA did not observe any objects that could be hazardous to children in care. The are no pools, spas or other water features. There are no pets currently in the home.
Children\342\200\231s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents\342\200\231 Rights, and documentation of 15-minute Infant Sleep Check (0-24 months)
LPA reviewed facility records including; LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan.
Licensee was reminded that fire extinguisher needs to be serviced yearly. The home maintains telephone service via lan line and cell phone.
Licensee was reminded to report any unusual incidents including changes to the household and if there are changes to areas used by children. --- Page 2 - Report Continues

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.
During the exit interview, Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
There are no deficiencies cited on this date. A notice of site visit was given and must remain posted for 30 days.
