What the inspector wrote

Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced Random Annual Inspection on this date. Upon arrival at 1:10pm LPA met with Staff 1, Beatriz Alvarez, who allowed entry into the home. LPA observed Staff 1 enter an off limits bedroom with door closed and bring out Child 1 (Infant age 5 months), At 1:11 LPA observed crib where infant was napping to have that had blanket and a bumber on two sides, posing an immediate risk to the health and safety of children in care. Also present was Adult 2. LPA toured home and observed 5 children paying in the backyard enclosed play area. At 1:14 LPA observed in Den 4 children present including 1 infant napping in a play yard with blanket, posing a potential risk to children in care. The licensee arrived at approximately 1:15pm, LPA explained the purpose of inspection and provided the inspection Entrance Checklist, LIC 126. All adults present have been cleared and associated to the facility. Individuals residing in the home were discussed and noted. At approximately 1:17pm LPA inspected the open kitchen (accessible to children and observed cleaning compounds in unlocked cabinet under the sink, the licensee removed them and placed them in off limits garage during this visit. LPA informed licensee that due to time limitations, this inspection will be continued at a later date. LPA discussed reporting requirements and capacity limitation of the license. Deficiencies cited during this visit need to be corrected to ensure the health and safety of children in care. LPA discussed Safe sleep regulations and proper care and supervision. Because the home is sectioned off into areas for play (outside in backyard) and napping (in den about 20 feet from den). The licensee was reminded to ensure a staff member is always close to den when infants are napping. Page 1 - Report Continues One (1) A Deficiency and Three (3) B deficiencies are cited during this inspections. When An A Deficiency is cited, the licensee will do the following: - A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. -A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). - A Signed copy of for LIC 9224 Acknowledgement of Receipt of Licensing Report must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the LIC 9224 Acknowledgement of Receipt of Licensing Reports Form during this visit. A copy of the Parent Notification Requirements was also provided to the licensee/Director. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were discussed and provided. Exit interview conducted and report was reviewed with the licensee ITA FANSHAW.
