What the inspector wrote

Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced Plan of Correction visit to follow up on previous Annual inspections conducted on 11/13/25 and 12/3/25 where deficiencies were cited. Upon arrival at 2:30pm LPA met with Ita Fanshaw, Licensee and Informed for the purpose of inspection. Also present was Daniel Dobens, Assistant, there were 6 children present during inspection. All adults present have obtained the required background clearance and are associated to facility. Individuals residing in the home were discussed and noted. During this visit LPA corrected previous deficiencies. LPA observed infants in care receiving proper care and supervision. Play yards are free from loose items, crib was observed in locked inaccessible bedroom, not used by daycare children. LPA reviewed facility files and observed Staff1 has proof of immunizations against Measles and Pertussis. Per licensee, immunization records for Child # 2 and 6 have not been obtained due holiday scheduling. Licensee requested to extended due date to January 31. No deficiencies were cited during this visit. Licensee was reminded of their responsibility to stay informed regarding childcare regulation to ensure the health and safety of children in care. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Ita Fanshaw.
