What the inspector wrote

CASE MANAGEMENT INSPECTION CONDUCTED On January 16, 2026, at 12:40 PM, Licensing Program Analyst (LPA) A. Carter conducted an Unannounced Case Management inspection to follow up on an incident that was reported to the Department on 1/14/26. Upon arrival, LPA met with facility representative Shirley Jones, who provided LPA on a tour of the facility Census was taken. On 01/14/26, an incident was reported to the department where a child fell and hit the table. The facility reported this incident to the Department within the required 24 hours. During this inspection LPA interviewed staff who observed incident and reviewed video footage. LPA obtained and reviewed documentation. Based on the information obtained and LPA observation of video footage of the incident, LPA determined there was adequate supervision provided the day of the incident. No deficiencies will be cited today 01/16/26. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview was conducted and report was reviewed with facility representative, Shirley Jones.
