What the inspector wrote

On October 29, 2025, at 8:35 AM, Licensing Program Analyst (LPA) A. Carter arrived at the above mentioned facility to conduct an Unannounced Case Management Inspection. LPA was granted entry into the facility by Facility Representative Ms. Jessica. LPA observed 4 children with 2 staff receiving proper care and supervision. At 9:10 AM, Facility Representative Shirley Jones arrived, LPA disclosed the purpose of the inspection visit. The purpose of this visit is to amend a Complaint Report completed by LPA Keneisha Dunlap, dated 07/24/25. The original report was amended on 10/24/25 by LPA Dunlap to change Complaint findings from Substantiated to Unsubstantiated . No deficiencies will be cited today 10/29/25. Exit interview was conducted and report was reviewed with the Director, Shirley Jones.
