What the inspector wrote

On 5/20/26 Licensing Program Analyst (LPA) Alicia Mooberry arrived at the facility above and conducted an unannounced Case Management inspection regarding an incident that occurred on 8/21/25 involving Child #1. LPA met with Facility Representative Karen Robles, Site Supervisor and, explained the purpose of the visit. LPA observed 28 children supervised by 11 staff. During this visit LPA reviewed documents, recorded observations and conducted interviews with Facility representatives. Myra Argueta, Program Manager arrived during this visit. LPA reviewed facility files within licensing department and did not find a record showing the incident was reported to the department within the required 24 hours by phone nor a written report LIC 624 which is required within 7 days of incident. Which poses a potential risk to the health and safety of children in care. Incident was reported by phone to the department on 5/19/26 and the written report LIC 624 was received by the department on 5/20/26. One B deficiency is cited during this visit in accordance with Title 22 regulations. Exit interview was conducted with Karen Robles, Site Supervisor. A copy of this report was provided. Appeal rights were discussed and provided. A Notice of Site Visit and copy of the report was issued. Notice of Site Visit must remain posted for 30 days. Failure to do so will result in a $100.00 civil penalty.
