What the inspector wrote

On 03/01/2024 at 12:30 PM an unannounced Required \342\200\223 3 Year Inspection was conducted by Licensing Program Analysts (LPAs) Katrina Chicote and Anthony Padilla. Licensing staff announced purpose of visit and met with Facility Representative (FR), Patricia Brizuela, Center Manager, who granted entry to facility and provided guided tour of facility both indoors and outdoors. The program currently operates an Infant Program with a Toddler Option Monday thru Friday 7:45 AM to 3:15 PM. LPAs observed there are separate entrances for each program and each program maintains it's own space. LPAs discussed required licensing postings with Facility Representative. At the initial start of the site visit, there is a total census of 18 infants and a total census of six staff present. The facility is made up of ten classrooms but, due to low enrollment, currently only using four classrooms: RM-1 (Infants between 18-24 months), RM-2 (Toddlers: 24-36 months), RM-7 (Infants), and RM-8 (Infants). LPAs observed RM-1 to have eight infants with two teachers, RM-2 to have five toddlers with two teachers, RM-7 to have five infants with two teachers, and RM-8 to be empty. Per FR, RM-7 and RM-8 combined on this date due to low attendance. Beginning at 12:30 PM, furniture and equipment were inspected for age appropriateness, good repair, and free of loose and sharp pointed parts in each classroom. Telephone service, heating, lighting and ventilation were evaluated. LPAs observed surface of classrooms to be safe and clean, and age appropriate tables and chairs available for infants well maintained. LPAs observed a variety of age appropriate material and equipment, including washable toys available for infants in each classroom. LPAs observed toys available for infants encouraging auditory, visual, and tactile stimulation as well as manipulative skills. LPAs observed library area available for quiet play in each classroom. There were cubbies observed as storage for infants' belongings. LPAs observed low feeding chairs available for infant/toddler feeding. LPAs arrived during infants/toddlers nap time and observed all infants and toddlers sleeping on cots that do not hinder walkways Report Continues - Page 1 of 4 or exits/entrances at time of inspection. LPAs did not observe infants sleep space to be separate from activity space. Per FR, infants/toddlers currently enrolled are over 18 months at this time and have scheduled nap times but should there be an infant that falls asleep outside of designated nap time they will create a plan to ensure that an infant who is sleeping is kept separate from activity space by a physical barrier. Per FR, linens are provided by the facility and washed weekly while blankets are provided by parents and taken home weekly. Availability of drinking water was reviewed. LPAs observed water coolers available with paper cups in each classroom. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPAs observed bathrooms available in between classrooms that have operable equipment, adequate supplies, and to have general area safety and sanitation. LPAs observed walk up diaper changing tables available in each classroom that is within arms reach of a sink with raised sides at least three inches high and with one inch padding surface that is covered with a removable paper at time of inspection. Snack/lunch menus were reviewed and observed posted in each classroom at time of inspection. All meals and snacks are provided Lynwood Unified School District Kitchen and distributed by facility staff. Infants/Toddlers are provided with breakfast, lunch, and snack. LPAs observed trash cans available in each classroom with tight fitting lids at time of inspection. A first aid kit is kept in a plastic bin on high shelf in each classroom and was inventoried for supplies . Per FR, facility has dual carbon monoxide detectors and smoke detectors, LPAs were not able to test carbon monoxide detectors at time of inspection due to nap time but observed a light indicating operation. LPAs observed detectors and fire extinguishers present in each classroom. LPAs observed disaster drill log available, last drill conducted on 02/14/2024. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. LPAs observed cleaning solutions to be stored on high shelves in each classroom and under sink cabinets with operable child safety locks. FR states that there are no poisons to kill vermin stored at the facility. LPAs observed medications for C1, C2, and C3 stored in back packs in classroom out of reach of children and according to Title 22 Regulation requirements. Teacher-Infant ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of infants/toddlers were observed by LPAs. Report Continues - Page 2 of 4 LPAs observed outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed a designated Infant Play Yard available that is separate and one shared Outdoor Play Area for use for Toddler Program. Required shade and fencing were inspected. LPAs observed shaded area through out and an age appropriate play structure that has a surface well maintained to absorb fall at time of inspection. LPAs advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to a watercooler with paper cups for outdoor play. LPAs observed water cooler available at time of inspection. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection. Staff and children\342\200\231s records were reviewed. Children\342\200\231s Records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857. The review of Staff records was documented on the LIC 859. LPAs discussed staff having current AB 1207 Mandated Reporter Training certificate on file. LPAs observed Pediatric First Aid/CPR expires on 07/2024 for Staff #2. All staff and employees have obtained a criminal record clearance or criminal record exemption as condition of employment with Lynwood Unified School District. LPAs did not review confidential files at time of inspection due to files being stored at Administrative Office which includes: LIC 503 and Immunization Records. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. Report Continues - Page 3 of 4 Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPAs advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation. The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited only advisories given today, 03/01/2024. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the Facility Representative, Patricia Brizuela.
