Upon the occurrence, during the operation of the child care center...a report shall be made to the Department...within the Department's next working day and during its normal business hours... Any suspected physical or psychological abuse of any child. This requirement was not met as evidenced by: Based on interviews and record review an incident that occured on 1/20/26 reporting Staff 1 pulled child's hair and tapped C3 on the hand was not reported to the department until 1/26/26 which poses a potential risk to the health and safety of children in care.

This poses an immediate risk to the health and safety of children in care.
