What the inspector wrote

On January 9, 2026 at 12:45 p.m., LPA Peter Bishop arrived at the above Facility for the purpose of a Case Management - Plan of Correction (POC) follow up of the Type A Deficiency cited on January 8, 2026. Upon arrival LPA Bishop met with Licensee Rosa Cruz a census was taken. At the time of this visit there were 2 children present and 13 enrolled. The Type A deficiency cited was uncleared adult in the home. This home is a one story home which consists of 3 bedrooms, 2 bathrooms, 2 Living Rooms, Kitchen and Dining Room. Due to the nature of the Type A Deficiency cited LPA Bishop looked in every room of the home to include all off-limits areas to verify and confirm that there were not any uncleared adults in the home at the time of this visit. LPA Bishop did not observe anyone in the home. LPA Bishop provided consultation to Licensee Cruz informing her of the importance of not allowing anyone in the home who has not been fingerprint cleared. LIcensee Cruz agreed and confirmed that she understood the importance of following all of the rules that are set forth. LPA Bishop explained the procedure that needs to take place regarding this deficiency. LPA explained that a copy of the report must be given to each parent. The LIC 9224 Acknowledgment of Receipt of Licensing Report must be signed by each parent that receives a copy of the report and maintained in every child's file. LPA Bishop further explained all children that are enrolled for the next year must be notified of the Type A Deficiency that was administered as well. LPA Bishop provided Licensee Cruz with copies of the LIC 9224. LPA Bishop collected a letter from LIcensee Cruz indicating that she is aware that noone is to be in the home caring for children that is not fingerprint cleared and she will not allow this to happen again. Licensee Cruz Page 1 of 2 provided a copy of the LIC 9163 Request For Live Scan Service had been completed for the individual that was observed in the home on January 8, 2026. The documentation indicates that the Request was submitted on January 9, 2026. The items provided is sufficient to clear the Type A Deficiency and all of the above instructions must be followed. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the Licensee Rosa Cruz. Page 2 of 2
