What the inspector wrote
On 4/13/23 Licensing Program Analyst (LPA) Jeanette Estrada conducted an announced pre-licensing inspection. The purpose of this inspection is to inspect and evaluate the facility for a new small license.
LPA met wit Licensee Beausalicia Diaz who provided a tour of the facility. Operating hours will be Monday to Friday 6AM to 5PM and care will be provided for children ages
3 months to 12 years old. All residents living in the home were discussed and verified and have obtained a criminal record clearance.
The day care facility is a 3 bedroom, 3 bathroom home with a front yard, living room, dining area, kitchen, backyard and attached garage (no access from inside the home).
Off Limit areas:
two bedrooms ( kept locked), two bathrooms (one bathroom in the home and one bathroom in the backyard), two sheds in the backyard and garage.
On Limit areas: Living room, hallway bathroom, dining area, kitchen (in passing to backyard), one bedroom and backyard. The day care areas that will be used by children were observed for safety, comfort, cleanliness, telephone service (cell phone), ventilation and heating. The facility has central air/heating. No wall heaters or fire places were observed. Per Licensee, children will have access to one bedroom that will be the daycare room and the living room as needed. LPA observed age appropriate materials and toys available for children. LPA observed that napping cots are available. LPA observed one crib and one play yard in the day care room. Per Applicant, food will be provided for children in care and children will be supervised while eating in the kitchen.Applicant was advised that if food is brought from the children\342\200\231s homes, the container shall be labeled with the child\342\200\231s name and properly stored or refrigerated. LPA advised applicant that high chairs are to only be used for eating during mealtimes. Detergents, cleaning compounds, sharp objects and hazardous items that can pose a danger to children are inaccessible to children in care. Knives and sharp objects are kept in an upper cabinet in the kitchen. Personal medicines are kept in a latched upper cabinet out of reach of children. Per Applicant, they plan to administer medication. Children's medicine will be kept in the upper cabinet.

Cleaning compounds are kept in latched cabinet under the kitchen sink. Laundry detergent is kept in the locked laundry room in the backyard. Per the Licensee, poisons are maintained in a locked shed in the backyard. LPA advised that if any poisons are kept in the home at a later date to keep them in a locked cabinet or container. LPA observed an operable smoke/carbon monoxide detector in the day care room. LPA observed a 2A10BC fire extinguisher in the kitchen with a receipt for proof of purchase in 1/2023 . LPA observed electrical outlets covered throughout the home. Children will use the bathroom in the hallway. LPA observed the cabinets had latches installed where personal items are stored. LPA did not observe any medicines in the bathrooms.
Children will have access to all of the backyard. Per applicant, children will walk through the kitchen to access the backyard. Per applicant, children will be supervised while playing outside. In the backyard LPA observed 7 rose bushes. LPA advised rose bushes will require to be inaccessible to children in care.
Per the Applicant there are no bodies of water on the premises such as pools/ hot tubs or spas. LPA did not observe any bodies of water. Applicant stated there are no weapons or firearms in the home. Per applicant there are no pets in the home. Per Applicant, if a child becomes ill, they will be taken to another on limits section of the home where they can be supervised.
Applicant has completed the required Health and Safety Training, Nutrition Training, Lead training and Pediatric First Aid and CPR issued by the American Red Cross which expires on 4/1/2025. Applicant has also completed the required Mandated Reporter Training. Applicant has proof of the required immunizations for measles, pertussis and has declined the flu vaccine. Applicant and adults living in the main facility have submitted proof of TB clearance.
LPA discussed the safe sleep regulations with Applicant. Applicant stated they plan to enroll infants. Infants will nap in the day care room. The crib does not block the entrance to the day care room. Per Applicant, the door will remain open while infants nap. LPA advised Applicant that there shall be no loose objects in the cribs. LPA advised Applicant that high chairs shall be used for their intended use and children shall not be left in car seats. LPA discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the

United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Incidental Medical Services (IMS) policy was discussed. At this time, the Licensee does not have any children enrolled that require medication. For IMS information , see

LPA reviewed with Licensee the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted.
Entrance Checklist was provided to the applicant.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
The following items are required to be corrected before the application is submitted for Department review:
-Proof that rose bushes (7) in the backyard have been made inaccessible. Per Applicant will be submitted by 4/28/23.
A small family child care license will be granted upon Department approval. Once licensed, the Licensee is required to comply with the terms and limitations stated on the license. Exit interview conducted and report was reviewed with Applicant Beausalicia Diaz
