What the inspector wrote

Licensing Program Analysts (LPAs) conducted an unannounced Case Management inspection at the above facility. At 11AM LPAs met with Licensee Kehana Lewis explained the purpose for our visit and were guided on a tour. LPAs observed 5 children in care including 2 infants . Also present during the inspection was Assistant Brisa The purpose of today’s visit was to follow up on an unusual incident reported to LPA on 5/21/26. Per License no police were present at her FCCH. Per Licensee her biological children were removed from her care and placed with her parents due to a separate ongoing DCFS matter. LPAs explained to Licensee reporting requirements to which she stated her own personal matters do not need to be reported to Licensing. LPAs again explained to Licensee reporting requirements. A type B Deficiency was issued for reporting requirements. A notice of Site Visit was issued and shall remain posted for 30 days. Exit interview was conducted with Licensee Kehana Lewis, a copy of this report as well as appeal rights were provided .
