What the inspector wrote

Licensing Program Analysts (LPAs) Jeanette Estrada and Tyler Reyes conducted a case management inspection for the purpose of evaluating the facility for a large license. LPAs met with Licensee Rossana Landeros and informed her of the reason for the visit. There were 4 children present upon arrival. Three additional children arrived at 3:35 PM. Present during today's inspection were Licensee and Assistant 1 who also resides in the home. Both adults present have obtained a criminal record clearance. The facility provides care to children ages 0 to 12 years old and is open Monday to Friday 12:00AM to 11:59 PM. Licensee was reminded care cannot be provided for a continuos 24 hours. An approved fire clearance for a capacity of 14 was issued on 3/10/25. This is a one story 3 bedroom, 2 bathroom home with a living room, dining room, kitchen, enclosed backyard and detached garage. Per Licensee areas on limits to children are: The living room/ dining room, kitchen (for passing to yard), one hallway bathroom and side yard. At the time of inspection, Licensee requested for the second restroom to be added as on limits. Areas off limits to children: Three bedrooms and enclosed backyard. All areas that are accessible to children were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection: LPA observed required posted documentation- Facility license, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. There are toys and other age appropriate materials available for children to use in the day care areas. Licensee had a current copy of the facility roster and the last documented disaster drill was conducted on 3/7/25. There is an operable carbon monoxide detector and an operable smoke detector in the living room. LPAs observed a pull down fire alarm and a 2A10BC Fire Extinguisher in the facility which indicated full and had a service tag dated 7/25/24. The home maintains telephone service (cell phone). Food is provided by Licensee. Licensee was reminded if children bring food from home it must be labeled with the child’s name and properly stored or refrigerated. Sharp objects, detergents and cleaning compounds were inaccessible to children in care. Page 2 Knives are kept in the back of a drawer in the kitchen. Although the kitchen is on limits, there is a safety gate separating it from the on limits day care area. Per Licensee, children enter the kitchen with supervision to access the on limits yard. Cleaning compounds are kept in an upper cabinet in the hallway inaccessible to children. Personal medication is kept in a locked box above the refrigerator. Per Licensee, only one child currently enrolled keeps medicine at the facility. Per Licensee, it is maintained in the children's cubby area out of reach of children. Laundry detergent is kept in the off limits garage. Children will have access to both restrooms in the home. LPAs inspected restroom 2, not previously on limits. LPA did not observe any hazardous items in the restrooms. There is no medicine in the restrooms. LPAs observed personal items stored in the restroom. Infant children are changed on a diaper table in the living room. The isolation area is located in the living room/dining room area separated from other children. Children use the enclosed side yard for outdoor play. There is a gate separating the on limits section from the off limits section. Per Licensee, children are supervised while playing outside. Per Licensee, poisons are contained in the garage. Licensee was advised that all poisons must be locked, not only inaccessible to children. Per Licensee, there are no firearms/ weapons in the home. There are no pools or spas, or other bodies of water. Per Licensee there are two dogs kept in the off limit area of the yard. Currently licensee cares for one infant. LPA observed one play yard in the living room. LPA observed that napping equipment does not block entrances or exits. Infant mattresses were observed to be firm with tightly fitted sheets. Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement acknowledging requirement to report child abuse and Mandated Reporter Training Certificate. Licensee has a pediatric first aid/ child cpr certification issued by the American Red Cross which expires on 1/10/27. Licensee and Assistant 1 completed Mandated Reporter training on 3/12/25. Licensee was advised at least one staff member must have a current pediatric first aid/cpr certification at all times. Both staff have proof of required immunization against pertussis, measles and flu and proof of TB clearance. Licensee was reminded that if an assistant provider is not available, the facility must follow capacity requirements for a small license. During inspection all children were observed to be treated with dignity and respect, they were observed to be receiving safe, healthful and comfortable accommodations, furnishings and equipment, and free from corporal and/or unusual punishment. Page 3 A large family child care license will be granted upon Department approval. Once licensed, the Licensee is required to comply with the terms and limitations stated on the license. Exit interview conducted and report was reviewed with Licensee Rossana Landeros. Notice of site visit was provided and must remain posted for 30 days.
