What the inspector wrote

Licensing Program Analyst (LPA) Jeanette Estrada conducted an unannounced visit at the facility for the purpose of delivering findings for the allegation above. LPA initially met with Staff 1 - who was supervising 5 children. Licensee arrived a few minutes later. During the course of the investigation LPA conducted interviews with the Parent of Child 1, the Licensee, Staff, children and parents of children enrolled. LPA also reviewed video footage. Per the report provided to the Department, Parent of Child 1 noticed a bruise on Child 1’s face and back. Per interviews conducted with Licensee and Staff, on 2/14/25 Child 1 fell face forward onto a chair during a game of musical chairs while playing at the facility. LPA observed video footage showing this incident. There was no footage of any incident involving the child’s back. Licensee and staff deny seeing any bruises on the child’s back. Interviews conducted with children and parents of children enrolled revealed no concerns of safety about the care provided. Based on interviews and record review the above allegation is unsubstantiated meaning although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and a copy of the report were provided to Licensee Rossana Landeros .
