What the inspector wrote

Licensing Program Analyst (LPA) Ashley Calderon conducted an unannounced case management incident visit. LPA met with Licensee Rossana Landeros who allowed LPA entrance into the facility. LPA disclosed the purpose of today's visit. Upon entrance LPA Calderon observed Licensee, with 2 staff assistants and 10 children (two whom were infants). Individual adults were discussed, on the facility Guardian Roster and are fingerprinted cleared. Facility reported incident on 4/21/26, and incident took place on 4/1/26 when Child #1 (C1) seeked medical professional care due to injury that occurred at the above facility. Due to not reporting timely, deficiency was cited. LPA reviewed reporting requirements from Title 22; 102416.2 Reporting Requirements . LPA conducted a tour of the inside and outside of the facility alongside R.Landeros. LPA Calderon conducted interview with Licensee R.Landeros. Staff #2 who was present during incident of C1 was not at the facility during today's visit. C1 was not present during LPA's visit, per Licensee they come in the AM only and left for the day. Per LIC624 Written Unusual Incident Report, Incident took place in the side yard where LPA observed area. Plastic play structure was observed, although the area C1 jumped from causing them to self injury themselves had no defects, the play structure was observed at 2:13pm to have two cracked areas, one crack on the platform of the area next to the slide and the other is on the outer part of the pink tunnel; cracks can create safety risk for children as opening of cracks can cause entrapment hazard for fingers, clothing and skin and cracks observed to have rough/ jagged edges and cracks on play structure can create structural weakness, play structure observed to have hazardous instability, therefore deficiency was cited for unsafe condition of play structure. Picture taken of play structure. LPA observed there was an age appropriate play structure, toys and materials available for children inside and outside of the home. Per Landeros interview, they warned C1 not to jump from play structure, C1 continued to play and jump off structure again and that is when incident took place of child falling. Licensee stated after fall child was okay C1 continued to engage in activities as no injury was observed and it was during music and movement when Licensee Landeros, observed C1 not moving their arm and C1 parent was called and notified about incident of their child not moving their arm and suggested to take them to see the doctor. Licensee provided LPA Calderon with incident report that was given to C1 parent on the day of incident. LPA collected via email on 4/22/26 Doctor's note regarding C1. Doctor's note suggested no playground, balls and jumping. Licensee stated they talk to C1 to take it easy due to injury and C1 although does not listen, therefore Licensee has notified parent and parent is understanding of child's hyperactive behaviors. Licensee during interview stated we try to keep C1 entertained to avoid running like brining non active activities but C1 continues to be active and run. In addition during this visit this LPA reviewed per Licensee request for advice with: - Infant Safe Sleep 102425 and Activities for infants - due to Licensee having two infants in care requirement reviewed. LPA observed 1 play yard and two infants, per Landeros 1 infant does not sleep on a play yard but sleeps on a play mat due to preference of child and safety as they try to climb play yard with their foot. -Operation of a Family Child Care Home 102417 ( Physical plant) - during today's visit Licensee asked how to remove 2 on limit spaces to be off limits. During this visit LPA observed sections that Licensee reported wants them to become off limits to the children. The two off limits being requested is the back yard and 1 restroom. LPA requested Licensee to submit to LPA Calderon facility sketch for changes; Licensee submitted change on the Facility Sketch LIC999A during visit. - Immunization records 102418- LPA reviewed Regulation and PM286 (link provided) Based on California Code of Regulation Title 22, the facility was given (2) deficiencies. A notice of site visit of site visit was given and must be posted for 30 days. An exit interview was conducted with Licensee Rossana Landeros.
