ANNUAL / RANDOM INSPECTION CONDUCTED On April 27, 2026, at 9:40 AM, Licensing Program Analyst (LPA) A. Carter conducted an Unannounced Annual / Random Inspection and met with Jamie Flores, the licensee . LPA disclosed the purpose of the inspection and was granted entry into the facility. LPA was taken on a guided tour of the facility and provided licensee with a copy of the Facility Entrance Checklist. Adults in the home were discussed, and all have criminal record clearance. Also present was Licensee’s Assistant. There were 8 children present during today’s inspection and currently 14 children enrolled. The children's roster was reviewed by LPA and is current. Hours of operation are Monday through Friday from 7 AM to 5 PM. This is a single story home which consists of 2 bedrooms, 1 bathroom, and attached garage. Areas accessible to children include: the living room (main care), 1 bedroom, hallway bathroom. Areas off-limits to children and parents are: 1 bedroom, laundry room, attached garage. Kitchen is used in passing. The licensee acknowledged that children may never enter these off-limit areas. LPA observed a safety gate at the entrance of the kitchen, making area inaccessible. The licensee understands that licensing staff may have access to off-limit areas during inspection visits if necessary. All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. .........................................................Report Continues 1 of 5 Pages....................................................... .........................................................Report Continues 2 of 5 Pages....................................................... LPA observed a wall mounted A/C unit in the main care area. A working telephone is maintained in the home via cellphone. LPA observed a wall heated in the living room/hallway with a service tag showing the gas was shut off. Facility License, Emergency Disaster Plan, and Parents’ Rights were posted and observed at time of inspection. Disaster drill log was available during today’s inspection with last disaster drill conducted on 04/17/24. LPA reminded the licensee that disaster drills should be conducted and documented every 6 months, deficiency cited. LPA observed the required 2A10BC fire extinguisher with service tag dated: 04/16/25. LPA reminded the licensee that fire extinguishers should be purchased new or serviced annually, deficiency cited. Smoke and carbon monoxide detectors were observe and tested at time of inspection. First Aid kit is mounted on the wall in the main care area observed by LPA. LPA observed main care area has age-appropriate toys, free of loose and sharp parts. There is a large area rug. Napping mats were observed in the main care area. There are cubbies for the children’s belongings and child size tables and chairs. Children use the bathroom located in the hallway. LPA observed bathroom to have operable toilet/sink and adequate toileting supplies for children in care. Personal care items are located in a latched cabinet under the sink, inaccessible to children in care. No hazards observed in bathroom. LPA observed a safety gate at the kitchen entrance making it inaccessible to children in care. Food is provided for children in care. LPA reminded licensee that any food brought from the children’s homes shall be labeled with child’s name and properly stored or refrigerated. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible stored in off-limit areas of the home. Detergents and cleaning supplies are stored in the laundry room. Personal medication is stored in the locked hallway closet, inaccessible to children in care. The Licensee states that there are no poisons in the home and understands that poisons must be locked. .........................................................Report Continues 3 of 5 Pages....................................................... Per licensee, the children have access to the backyard for outdoor play. LPA observed yard has grass and adequate perimeter fencing through-out the property. LPA observed a variety of age appropriate toys available to children in care. Play area has shading and is free of loose and sharp articles or objects and all trees, shrubs, and plants are maintained. Infant Care: Currently there are no infants enrolled. The Licensee was advised; infants shall not be swaddled in care. Car seats shall only be used for transportation purposes and shall not be used for sleeping. LPA discussed the Safe sleep regulation, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0 -24 months. —Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15-minute Infant Sleep Check (0-24 months). All required licensing forms are in file. Staff records were reviewed: Licensee's Pediatric CPR / First Aid valid until: 03/03/27 and the Assistant's Pediatric CPR / First Aid valid until: 06/05/27. Staff had proof of required immunizations including proof of TB clearance. Staff had LIC 508- criminal record statement and LIC 9052- employee rights on file. Licensee's Mandated Reporter training is valid until: 04/21/27 and Assistant’s valid until: 06/05/27. The following was discussed: · Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers, or any other item that falls into that category are permitted in the facility. · Effective January 1, 2010, Licensees of family childcare homes are required to ensure that at least one staff member with current training in Pediatric First Aid & CPR is on site at all times when children are present. · Reporting Requirements: Changes should be reported to the Department as soon as they occur, such as construction, remodeling, telephone number changes and/or if you move from your home. Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing. .........................................................Report Continues 4 of 5 Pages....................................................... The licensee confirms: The isolation area for sick children waiting to be picked up is the main care area The facility does not currently administer medication. There is one pet dog in the home that does not interact with day care children There are no smokers in the home There are no weapons or firearms in the home There are no pools, spas or other bodies of water on the premises During the inspection, children present were observed to be treated with dignity and respect, observed to be receiving safe, healthful, and comfortable accommodations, furnishings, and equipment, and free from corporal and/or unusual punishment. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Jamie Flores, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative.
