What the inspector wrote

Licensing Program Analyst (LPA) Raul Navarro and Licensing Program Manager Kaaren Chambers conducted a teams meeting on 11/14/23 at 1:00 PM. LPA and LPM met with Applicant Vanessa Ortega. The purpose of today's teams meeting was to go over the items needing attention on the child care center application. Topics covered during the meeting are listed below: Based on my review of the above pending application a teams meeting will need to be scheduled with the applicant. The following items are in need of being addressed/corrected: LIC 200 applicant is Vanessa Ortega \342\200\223 should be under the LLC Kreative Kidz (this will also require the profile to be updated).Noelia Rosas \342\200\223 no finger prints.LIC401 monthly operating indicates 36 when applying for 42.LIC404 Financial information \342\200\223 not received from financial institution. LIC500 does not indicate all noted \342\200\223 job description, assistant director, supervisor. LIC610 \342\200\223 staff multi-tasking \342\200\223 missing permission from relocation sites. Relocation site in parent handbook not the same as LIC610. Appears the facility is providing potty training \342\200\223 how will they dispose of the diapers and how many potty chairs do they have? ( one for every five being potty trained). Where is the medication being stored? Nebulizer \342\200\223 staff administering must be trained by the parent and LIC9166 completed. IMS plan missing. Pandemic plan missing. Parent handbook states \342\200\234State law food brought from home must be store bought (pg12). Director qualifications \342\200\223 Vanessa Ortega is not qualified \342\200\223 no proof of \342\200\234family child & community\342\200\235 missing proof of experience as a teacher \342\200\223 experience/reference letters missing. Declination flu \342\200\223 missing for Vanessa Ortega. Hours for director shift don\342\200\231t match LIC500. Staff hours don\342\200\231t match LIC500. Employee handbook has staff signing children in\342\200\246why?. In-service-training \342\200\223 who\342\200\231s providing the training for each subject to be covered. Why is staff being trained quarterly in 1 st aid and CPR \342\200\223 who\342\200\231s providing this training?. Pg 24 of EE handbook references Nevada requirement \342\200\234not on duty if you have your cell phone\342\200\235 this does not apply to California All Job descriptions reference 6:1 ratio if this is so, not enough staff listed on LIC500. Job description references aides being able to supervise 6 children alone \342\200\223 aides cannot be left alone. Who is Kristen \342\200\223 EE handbook indicates aides are to turn in their curriculum to her every Thursday. Aide job description references bottles and changing. Floater line of supervision missing. Is the floater an aide or a teacher \342\200\223 if an aide can\342\200\231t be left alone. Directors job description references an Evelyn \342\200\223 Director is to confer with her. Directors job description no mention of experience/education/chain of command. Supervisor job description \342\200\223 no mention of experience /education or chain of command. None of the job descriptions mention the requirement of finger print clearances, immunization. Parent handbook reference toddlers \342\200\223 not applying for a toddler option. Daily schedule missing. Parent handbook \342\200\223 requirement for health check prior to signing in missing. Required forms for parents missing (list or actual form). Daily schedule missing including nap. Modification condition \342\200\223 missing. Refund policy missing. Rights of Licensing missing. Sample menu with portions missing. Mandated reporter training \342\200\223 references Nevada Law and phone number. Car seat/transportation \342\200\223 not in compliance with California Law. Handbook states while children are napping \342\200\223 can be in bathroom cleaning \342\200\223 who\342\200\231s watching the children napping ( all will not be asleep). State of California does not consider on/off duty if phone is in hand \342\200\223 this is not a part of CCL requirements. What agency is this from?. Mention of breast milk /formula \342\200\223 this is a preschool program. Most of the items on the list have been corrected and were discussed during the meeting. Per Applicant, they will try to have the remaining items completed by November 16,2023. Exit interview was conducted with Applicant Vanessa Ortega.
