On January 23, 2026, at 9:05 AM, Licensing Program Analyst (LPA) A. Carter conducted an Unannounced Case Management Inspection. LPA announced purpose of inspection and was allowed entry into the facility by Janice Balgemino, facility representative(Director). The purpose of this visit is to change findings and remove a citation issued on Complaint report dated 09/30/25. The original report was amended to change Complaint allegation findings from Substantiated to Unfounded. No deficiencies will be cited today 01/23/26. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview was conducted and report was reviewed with facility representative, Janice Balgemino.
