What the inspector wrote
On 05/01/25, at 1:10pm, Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced Annual Random inspection and met with Licensee, Tesera Perez. LPA disclosed the purpose of the inspection and was granted entry unto the facility by the Licensee. There were three children upon arrival. Current hours of operation are Mon-Fri 8:00 AM - 5:00PM. But is available 24/7 if needed. Facility capacity is in compliance for a Large Family Child Care Home.
This is a single story home that consists of four bedrooms, two bathrooms, kitchen, dining room, living room, daycare room, and attached garage. Per Licensee and Facility Sketch, the children will have access to kitchen (located to the left upon entry), dining Room (located through kitchen), living room (next to dining room), bathroom 1 (located down hallway to the right), daycare Room (located to the rear of the home). Off limit areas of the home will be all bedrooms and bathroom 2 (which is located inside bedroom). Licensee states off limits bedrooms and garage will be locked during operating hours, and LPA observed bedroom doors to have locks available at time of inspection.
LPA reviewed required posted documentation for Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. Facility records were reviewed for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log, last drill conducted in 05/05/25. Smoke and carbon monoxide detectors were tested and operable. Fire extinguisher indicated fully charged and was serviced on 04/24/25. The home maintains telephone service via the Licensee's cell phone. The home is observed to be clean and orderly.

LPA observed that detergents, cleaning compounds and medication are inaccessible to children. Licensee understands that all poisons must be lock, not only inaccessible to children. Isolation area for sick children waiting to be picked up is in the dining room area, away from the other children. Per Licensee there are no firearms or weapons stored in the home. Licensee has a medium size dog and a small dog.
Currently licensee cares for one infant, however, that infant will turn two by the end of the month. Infant sleeps on a mat. Napping equipment does not block entrances or exits. LPA observed 15 minute sleep check documentation for infants .
Children will use the backyard for outdoor play. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that could be hazardous to children in care. There is an in- ground pool that is double gated. LPA observed the following during inspection: LPA observed that the gate does self latch.At 2:20pm, LPA did not observe an alarm that, when placed in a swimming pool, will sound upon detecting an entrance into the water. The alarm shall be turned on and be in working condition during a facility\342\200\231s operating hours while the swimming pool is not in use. Licensee stated that since there is a double gate, she didn't think she needed a alarm. A technical violation was given. AB-2866: The licensee has a life ring with a minimum exterior diameter of 17 inches and a 12 feet rescue pole visible from the swimming pool and readily available for immediate use.
Children\342\200\231s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents\342\200\231 Rights.
Staff records were reviewed for approved Pediatric First Aid and CPR certification (expires 04/10/26), LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate (expires 04/11/26).

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
During the exit interview, the LICENSEE T. Perez that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee T. Perez.
