What the inspector wrote

Licensing Program Analyst (LPA) Peter Bishop conducted an unannounced case management inspection at the above facility on 05/21/2026 at 1:15 p.m. LPAs met with Director Sara Ramirez. There were no children and present when LPA arrived. Director Sara Ramirez indicated that today is the last day of school. The purpose of the inspection was to address an Unusual Incident Report that was received in the licensing office on 04/23/2026. The date of the incident was 04/22/2026. During today's inspection LPA spoke with Director Sara Ramirez. Director indicated that the child was running on the rubber cushion mat and tripped and fell on her left arm, ice was applied. LPA Bishop observed the area where the child fell. LPA did not see any hazards that may have caused or contributed to the fall. The cushion was free of debris and damage. Director Sara Ramirez stated that there was not swelling initially but, parents were called and she did require a cast be placed on her arm. The child was allowed to attend school on 4/28/2026 and the cast will remain on her arm for a total of 6 weeks. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Sara Ramirez. Page 1 of 1
