What the inspector wrote

Licensing Program Analysts (LPA) Dayna Chambers conducted an unannounced case management inspection to follow up on an incident that was reported to the Department on 06/20/25 for an incident that occurred on 06/18/25. Upon arrival, LPA met with Director Anna Martinez who provided LPA with a tour of the facility inside and outside. Census was taken. LPA observed a total of 29 children, 5 staff and also Director. There are four classrooms: Guppies (18months to 36 months) with 7 children and 2 staff, Sea Horses (3yrs-4 yrs) with 9 children and 1 staff, Penguins (2yrs \342\200\223 3 yrs) with 8 children and 1 staff, and Dolphins (4yrs-5 yrs) with 9 children and 1 staff. There are currently a total of 44 children enrolled in the center. On 06/20/25, an unusual incident report that was self-reported to the department by Anna Martinez, Director. On 06/18/25 at approximately 10:25am, Child #1 and Child #2 were in the Sea Horses when the incident occurred. The bookshelf was on roller wheels at the time and child#1 pushed over the bookshelf and struck child #2 in the middle of her face. Child had some swelling and black eye. The director administered an icepack, comforting, and parents arrived quickly. Child #1 and Child #2 are still enrolled. The Director has removed all wheels from the bookshelves, and they are secured to ensure the safety of children in care. Witness Staff #1 contacted Director Anna Martinez. Incident were reported to the Department within the required 24 hours, and the written report was submitted within 7 days. Facility assessed the incident, and Director changed the bookshelves by removing the wheels and made the book shelve stable. The staff supervise children during incidents. During this visit, LPA discussed the toddler component 18-26 months, napping ratio, and sleep charts. The Exit interview was conducted with Director Anna Ramirez. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
