What the inspector wrote

On 1/17/2025 at 10:30 am, Licensing Program Analysts (LPAs), Elka Chavez and Crystal Green conducted an announced Pre-Licensing inspection for a Change of Location. Upon arrival, LPAs met with Applicant Representative Ashley Wiley. Applicant, Los Angeles Speech and Language Therapy Center, Inc, has requested to relocate from their existing licensed Preschool and Toddler Programs facility number 198400491, located at 3654 E. Imperial Highway, Lynwood, CA 90262. The Applicant is requesting to serve 25 children: (14) Toddlers ages 18 to 36 months in the toddler classroom and (11) Preschoolers from 2 to 5 years old in the preschool classroom. Hours of operation will be Monday to Friday from 7:30 am to 3:30 pm. The toddler program will operate from 9:00 am to 12:00 pm. This facility is located on the premises of Brister Memorial Baptist Church. Parents will have access to the facility via Long Beach Boulevard through the gated entrance facing the parking lot. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Ashley Wiley that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Ashley Wiley stated that they will be in kept in a locked box in snack area. LPAs did not observe first aid kit. Applicant stated that they will be kept in the snack area. There is an operational carbon monoxide detector on site located near the main entrance. All required licensing documents were observed posted in the main entrance. Children will be signed in and out at the main the entrance. LPAs continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured 521.64 square feet, which is sufficient to accommodate the requested capacity of 14 toddler Page 1 children. Total indoor activity space measured 398.48 square feet, which is sufficient to accommodate the requested capacity for preschool children. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, bookshelves, and other activity supplies for the children. LPAs did not observe cots or napping equipment. LPAs observe stairs that lead to the second floor are accessible to children. Applicant Representative stated that a child safety gate has been ordered. Drinking water is available in the snack area via water dispenser located in the snack area. There are no bodies of water or weapons on the property. Fire clearance was granted, the Department has not been able to obtain a copy of the approved fire safety inspection report from the Compton Fire Department. LPAs observed a total of 2 toilets and 1 sink for the preschool children and 1 toilet, 1 urinal and 1 sink for the toddler children\342\200\231s use. These are sufficient to accommodate the requested capacity of 11 preschool children and 14 toddler children. LPAs observe the women\342\200\231s restroom is designated for the preschool program and the men\342\200\231s restroom is designated for the toddler program. LPAs observed the toilets and sinks are not age appropriate. There is a separate staff restroom equipped with a toilet and a sink located within the church area. The isolation area for children who are ill will be the director\342\200\231s office. Facility will provide snack and parents will need to provide lunch. The snack area currently includes a microwave and a fridge. The snack area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. The facility currently has a fully fenced playground area designated for the preschool component. Fencing is wrought iron and is at least four feet high. The total square footage for all the outdoor activity space is 652 square feet, for only 8 children at one time, which insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage and shared use by the toddler children shall be submitted and approved prior to licensure. LPAs did not observe shade in the outdoor activity area. Applicant Representative, Ashley Wiley stated that shade will be provided via a tent that will be brought out by staff every morning. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. Drinking water is available via zippy cups filled using the water dispenser. LPAs did not observe hazardous items on the playground to be accessible to children. Applicant Representative, Ashley Wiley was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Page 2 For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Facility representative, Ashely Wiley was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. This facility plans to provide Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Page 3 The following corrections are needed prior to the issuance of the license: \342\200\242 Applicant needs to submit proof of the stairs leading to the second floor as inaccessible. \342\200\242 Applicant needs to obtain the necessary equipment to make the toilets and sinks age appropriate. \342\200\242 Applicant needs to submit proof of obtaining shade for the outdoor activity area. \342\200\242 Applicant needs to submit proof of a first aid kit. \342\200\242 Applicant needs to submit proof of cots obtain. \342\200\242 Applicant needs to update the Application (LIC 200A) form. \342\200\242 Applicant needs to provide board resolution designating the facility representative. \342\200\242 Applicant needs to update the Personnel Report (LIC 500) form. \342\200\242 Applicant needs to provide proof of Preventive Health Practices Training (with lead and nutrition) and proof of immunization's proof of Measles, Pertussis, and Influenza (decline declaration for influenza) for Ashley Wiley. \342\200\242 Applicant needs to provide proof of MMR for Pamela Wiley. \342\200\242 Applicant needs to provide Earthquake Preparedness Checklist (LIC 9148) \342\200\242 The Department must receive the Fire safety inspection report stating fire clearance has been granted. Applicant Representative, Ashley Wiley understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
