What the inspector wrote

On June 10, 2025, at 1:20PM, Licensing Program Analyst (LPA) Dayna Chambers, LPA was attempted to conduct the prelicensing visit with applicant Marquia Tyler. Applicant is applying for a small family home. Applicant had an emergency and had to leave. LPA will reschedule the prelicensing visit on June 23, 2025. Exit interview was conducted with applicant, Marquia Tyler. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
