What the inspector wrote

On August 7, 2025, Licensing Program Analysts (LPAs) Naves and Van conducted an unannounced continued case management inspection at the facility today. LPAs were there to conduct additional interviews pertaining to an unusual Incident reported to the department that occurred July 21, 2025. LPAs met with the site Director, Lexi Hallum, and assistant director Debora Osorio and the purpose of the inspection was explained. Present during the inspection were 24 preschool-age children, accompanied by five teaching staff. The facility was operating within its licensed capacity and within the required ratio today. The incident was self-reported to the Department on July 28, 2025. The incident occurred on July 21 and involved a teacher, S1, who allegedly pushed C1 to the ground after being hit in the face by C1. During the inspection today, LPAs interviewed additional staff and determined the incident reported was an isolated incident. LPAs determined staff member did not maliciously push child down but rather in a motion to protect themselves as well as other surrounding children. LPAs reminded the Assistant Director the importance of providing staff with assistance when needed and being vigilant in providing the assistance timely. No deficiency was issued during the inspection. A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, and the report and was reviewed with Assistant Director Debora Osorio.
