What the inspector wrote

On January 27, 2026, Licensing program analyst (LPA) Naves conducted an unannounced case management visit due to an incident self reported by the center that occurred on 1/12/2026. LPA was greeted by director Diana Gonzalez the purpose of the visit was explained and was granted entry to the facility. 24 staff including Director supervised 66 children on this day. On 1/12/2026 it was reported that a child was left alone outside a classroom in the play yard while transitioning to lunch time. Child left outside was discovered when child knocked on the door from the outside. LPA visited location where incident occurred and interviewed staff. Per Director she had a meeting with staff and has implemented some changes. Effective immediately staff will transition children from outside to inside in groups of 6. First group will be escorted by staff to hand washing/bathroom, second group will follow for handwashing/bathroom. Final group will remain outside and follow once prior group has finished and will scan outdoor area to ensure all children are accounted for. See 809D for deficiencies cited today. Facility also reported an incident that occurred on January 13, 2026, at 11:15 a.m. A notice of site visit was given and must remain posted for 30 days. Appeal rights given. Exit interview conducted and report was reviewed with the Director Diana Gonzalez.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Based on observations and interviews, the center did not comply with the section cited above. The center did not provide sufficient supervision; child was left outside of classroom on playground.Child was discovered outside after child knocked on the door. Failing to provide adequate supervision to meet the needs of children in care poses a potential health and safety risk for them.
